Özet

Oldukça güçlü organik fosforlar olan paratiyon ve malatiyonu suisid amacıyla içilen iki zehirlenme vakası anestezi yoğun bakım ünitemizde izlenmiştir.Yoğun bakıma gelişte, Glasgow Koma skorları 3 olan hastaların, serum kolinesteraz düzeyleri normalin % 10'unun altında bulunmuştur. Paratiyon zehirlenmesi olan olguda yaygın cilt lezyonları gelişmiştir. Aktif kömür (4 kez), pralidoksim (PAM) (12 kez) ve atropin infüzyonu ile destek tedavisi uygulanan hastalar yoğun bakıma yatışlarından sonraki 25. ve 26. günlerde sekelsiz olarak servislerine gönderilmiştir. Her iki hastada, 150-250 mg sa-1'e kadar varan dozlarda atropin infüzyonu uygulaması ile toplam 120 gr. atropin uygulanmıştır ve 800 yataklı hastanemizin yaklaşık 6 aylık atropin stoku tükenmiştir.Biyolojik silahların gündemde olduğu günlerde izlediğimiz bu hastalar, bizde şu soruyu akla getirdi: Böyle bir olayda çok fazla sayıda hasta ile nasıl baş edebiliriz? Bu konuya dikkat çekmek istedik.

Abstract

Two intoxication cases with strong organophosphates, parathion and malathion were managed in our intensive care unit.Plasma cholinesterase levels of patients whose Glasgow Coma Scores were 3 at the time of accepting into the intensive care unit were founded under 10 % of normal level. The patient with parathion intoxication had dermatologic lesions. Treatment was included with oral activated charcoal (4 times), pralidoxime (PAM) (12 times), atropine infusion and supportive therapy. The patients were discharged on the 25. and 26. days after hospitalization.Total amount of 120 gr. atropin which is our hospital stock of six months was administered to the patients by IV infusion up to 150-250 mg h-1.On the days in which the biological weapons were the main topic, a question came to our minds; how can we manage numerous patients in such a case? In the case report we want to take attention to this point.
