Pnömoperitonyum ve hasta pozisyonun endotrakeal tüp kaf basıncı üzerine etkileri

Özet
Amaç: Genelde, anestezi pratiğinde trakeal kapiller kan dolaşımın etkisini azaltmak için kullanılan endotrakeal tüplerin kaf basınç değerlerinin 20 cmH2O altında tutulması hedeflenir. Anestezik gazların, özellikle de nitroz oksitin kaf içerisine diffüze olarak kaf basıncını artırdığı da gösterilmiştir. Bu çalışmada pnömoperitonyumun ve hasta pozisyonun nitroz oksit etkisi olmadan kaf basıncına etkileri araştırıldı.Gereç ve Yöntem: ASA I-II grubu 40 hasta prospektif olarak çalışmaya dahil edildi. Grup I'deki hastalar (n=20), CO2 pnömoperitonyum ve masa pozisyonu baş yukarı, sol yan ile laparoskopik kolesistektomi için opere edildi Grup II'deki hastalar (n=20) supin pozisyonda inguinal herni onarımı için opere edildi Anestezi indüksiyonunda pentotal, roküronyum ve fentanil, anestezi idamesinde ise, oksijen, hava ve sevofluran kullanıldı. Kaf basınçları; inguinal herni grubunda, entübasyondan hemen sonra, her 10 dakikada bir ekstübasyona kadar ölçüldü. Grup I de ise, buna ilave olarak kaf basıncı entübasyon sonrası, pnömoperitonyum sonrası-pozisyon verilmeden önce, pozisyon verildikten sonra ölçüldü.Bulgular: Laparoskopik kolesistektomi olgularında, pnömoperitonyum sonrası kaf basınçları, entübasyon sonrası döneme göre anlamlı bir şekilde yükseldi (p
Abstract
Aim: Common anesthetic practice is to keep endotracheal tube cuff pressure less than 20 cmH2O to limit effects on tracheal capillary blood flow. Others have shown that anaesthetic gases, especially nitrous oxide, may diffuse into the cuff thereby increasing cuff pressure. In this study, we investigated the effects of pneumoperitoneum and patient position on endotracheal cuff pressure, in the absence of nitrous oxide.Materials and Methods: Forty ASA physical status I and II patients were studied. Group I patients (n=20) underwent laparoscopic cholecystectomy under pneumoperitoneum with CO2 positioned with the head up and left tilt. Group II (n=20) underwent inguinal hernia repair in the supine position. Pentothal, rocuronium and fentanyl were used for anaesthesia induction, oxygen, air, sevoflurane and alfentanyl were used for anaesthesia maintainance. Cuff pressure was measured immediately after intubation and every 10 minutes until extubation. In addition, cuff pressures were measured in Group I after pneumoperitoneum and before and after patient positioning.Results: In laparoscopic cholecystectomy patients, cuff pressure increased after pneumoperitonium and decreased after patient positioning (p
