Özet
Amaç: Bu çalışmada, kristalloid ve kolloid sıvıların spinal anestezi sonrası işitme kaybına etkilerinin incelenmesi amaçlandı. Ayrıca, işitme kaybı ile spinal anestezi süresince gözlenebilen hipotansiyon arasındaki olası ilişki de değerlendirildi.Gereç ve Yöntem: Çalışma prospektif olarak, 18-60 yaşları arasında, spinal anestezi uygulanacak 60 hastada planlandı. Rasgele seçilen vakalar, 3 gruba ayrıldı ve çalışma tek kör olarak tasarlandı. Grup LR (n=20) Ringer Laktat solüsyonu (preoperatif 20 mL kg-1, intraoperatif 2 mL kg-1 sa-1), Grup NaCl (n=20) % 0.9 izotonik sodyum klorür solüsyonu (preoperatif 20 mL kg-1, intraoperatif 2 mL kg-1 sa-1) ve Grup GF (n=20) modifiye jelatin solüsyonu (Gelofusine) (preoperatif 10 mL kg-1, intraoperatif 1 mL kg-1 sa-1) solüsyonu verildi. Spinal anestezi 25G Quincke iğne kullanılarak uygulandı ve % 0,5 bupivakain 10 mg ve fentanil 25 µg intratekal olarak verildi. Odyogram preoperatif ve postoperatif 2. gün uygulandı. Hastalar işitme kaybı, hipotansiyon, baş dönmesi, bulantı ve kusma açısından değerlendirildi.Bulgular: İşitme kaybının total insidensi %11.6 olarak bulundu. İşitme kaybı 2 hastada tek taraflı, 5 hastada iki taraflıydı. İşitme kaybı tüm hastalarda düşük frekanslarda tespit edildi ve postoperatif beşinci gün normale döndü. Grup LR'de işitme kaybının insidensi diğer gruplardan yüksek olmasına rağmen, gruplar arasında anlamlı fark bulunmadı (p>0.05). Hipotansiyonu olan hastalarda, gruplar arasında istatistiksel olarak anlamlı işitme kaybı yoktu. Bulantı, kusma ve baş dönmesi açısından gruplar arasında fark bulunmadı.Sonuç: Farklı yükleme sıvılarının verilmesi ile spinal anestezi sonrası işitme kaybı arasında anlamlı bir ilişki saptamadık.
Abstract
Aim: The aim of this study was to investigate the effects of crystalloid and colloid solutions on postspinal hearing loss (HL). We also assessed any possible correlation between the HL and hypotension.Materials and Methods: We conducted a prospective study in 60 patients, ages 18-60 yr, under spinal anesthesia. The patients were randomized in a single-blind fashion into three groups: Group LR (n=20) received lactated Ringer's (preoperative 20 mL kg-1, intraoperative 2 mL kg-1 h-1), Group NaCl (n=20) received 0.9 % NaCl isotonic saline (preoperative 20 mL kg-1, intraoperative 2 mL kg-1 h-1), and Group GF (n=20) received gelatin polysuccinate 4 % (Gelofusine) (preoperative 10 mL kg-1, intraoperative 1 mL kg-1 h-1). Spinal anesthesia was performed by using 25 G Quincke needle and was given bupivacaine 0.5 % 10 mg and fentanyl 25 µg. Audiograms were performed preoperatively and 2 days postoperatively. Hearing loss, hypotension, dizziness, nausea, and vomiting were evaluated.Results: Overall incidence of the HL was 11.6 %. The HL was unilateral in 2 and bilateral in 5 patients. HL occurred within the low-frequency range and the hearing thresholds returned to normal by the fifth postoperative day. Although the incidence of HL for Group LR was higher than other groups, there was no significant difference between the groups (p>0.05). In the patients who had hypotension, any statistically significant HL was not detected between the groups. There was no difference among the groups with respect to the complications of nausea, vomiting, and dizziness.Conclusions: We didn't find any relationship between the postspinal HL and the various loading solutions.
