Tiroidektomi operasyonu sonrası ağrı tedavisinde preoperatif lornoksikam ve diklofenak uygulamasının postoperatif tramadol gereksinimi üzerine etkinliği

Özet
Amaç: Bu randomize, çift kör çalışmanın amacı preoperatif dönemde uygulanan 8 mg lornoksikamla 75 mg diklofenakın postoperatif ağrı tedavisindeki etkinliğinin incelenmesidir. Gereç ve Yöntem: Çalışmaya tiroidektomi operasyonu geçirmesi planlanan ASA I-II grubundaki 75 hasta randomize olarak 3 gruba ayrıldı. Grup L, D ve P’deki hastalara sırasıyla preoperatif olarak 8 mg lornoksikam, 75 mg diklofenak ve 3 mL % 0.9 NaCl uygulandı. Ekstübasyon sonrası dinlenim, boyun hareketi ve öksürme sırasında baş, boyun ve omuz ağrısı, hasta memnuniyeti, solunum sayısı ve derlenme kalitesi VAS ile değerlendirildi. Bu parametrelerden birinde dört ve üzeri VAS durumunda Tramadol 100 mg intramusküler olarak uygulandı. Hastalardaki ilk analjezik uygulama zamanı, maksimum ağrı skoru ve zamanı, cerrahi sonrası ilk 24 saatteki toplam tramadol kullanımı kaydedildi. İstatistiksel analiz için tek yönlü varyans analizi, ki-kare testi, Mann-Whitney U testleri kullanıldı. Bulgular: Demografik özellikler gruplar arasında benzerdi. Dinlenim, hareket ve öksürme sırasındaki boyun ağrısı skorları plasebo grubunda ilk iki saatte daha yüksek bulunmuştur. Hasta memnuniyeti skoru postoperatif ilk 4 saat içinde plasebo grubunda daha düşük bulunmuştur. İlk analjezik uygulama ve maksimum ağrı skoru zamanı Grup P’de daha kısa bulunmuştur. Postoperatif ilk 24 saatte uygulanan toplam tramadol miktarı, dinlenim sırasında tariflenen en yüksek ağrı skoru Grup P’de daha yüksek bulunmuştur. Sonuç olarak, tiroidektomi sonrası ağrı tedavisinde lornoksikam ve diklofenak için benzer etkinlik bulunmuştur.
Abstract
Aim: The aim of this randomized, double-blinded study is to evaluate the effectiveness of preoperative lornoxicam 8 mg with diclofenac 75 mg in postoperative pain management. Material and Method: 75 ASA I-II patients undergoing thyroidectomy operations were randomly allocated to 3 groups. Preoperative lornoxicam 8 mg, diclofenac 75 mg and % 0.9 NaCl 3 ml were given to Group L, D and P respectively. Headache, neck and shoulder pain, patient satisfaction scores, respiratory frequency and recovery quality at rest, during neck movement and coughing were evaluated according to Visual Analog Scale (VAS0-100 mm). Intramuscular tramadol 100 mg was given when VAS score in one of these parameters was ? 4. The time of first analgesic requirement, maximum pain score, the time of maximum pain score, total tramadol requirement during postoperative 24 hours were recorded. One-way analysis of variance, chi square test and Mann-Whitney U test were used for statistical analysis. Results: Demographic data was similar in all groups. Neck pain scores at rest, during movement and coughing was found to be higher in placebo group at first 2 hours. Patient satisfaction score was lower in placebo group during postoperative 4 hours. The time of first analgesic requirement and maximum pain score was shorter in Group P. Total amount of tramadol requirement during postoperative 24 hours and maximum pain score at rest was higher in Group P.
