Özet
Obstrüktif uyku apne sendromlu hastaların cerrahisinde anestezi, ventilasyon ve entübasyonun güç olması, postoperatif dönemde solunum problemleri görülebilmesi nedeniyle zordur. Bu olguda uygulanan anestezi deneyimi sunulmuştur.61 yaşında, ağır obstrüktif uyku apne sendromlu, regüle hipertansiyonu olan erkek hasta septoplasti nedeni ile operasyon programına alındı. Anestezi indüksiyonunda tiyopental 5 mg/kg-1 yapıldı, ancak maske ile ventilasyonun sağlanamadığı görüldü. Bunun üzerine intralaringeal maske hava yolu (İLMA) kaşığı (fastrach no: 5) yerleştirilerek ventilasyon sağlandı. Remifentanil 1 µgkg-1, vekuronyum 0.1 mgkg-1 yapıldıktan sonra "fastrach" içinden entübe edildi. Anestezi idamesinde sevofluran % 1 ve % 50 oksijen içinde azot protoksit kullanıldı. Operasyon süresince hemodinamik bulgular stabil seyretti. Ekstübasyon sonrası üst hava yolunda kollaps gelişen hasta entübe edildi ve yoğun bakım ünitesine alındı. Postoperatif 4. saatte ekstübe edilen hasta, 24. saatte servise alındı.
Abstract
Anesthesia in surgery of patient with obstructive sleep apnea syndrome isn't easy, because of respiratory problems in postoperative period, difficulties of ventilation and entubation.A 61 years old, male patient, who had a obstructive sleep apnea syndrome, scheduled for septoplasty. He had regulated hypertension. Anesthesia was induced with thiopental 5 mg kg-1 but ventilation couldn't be maintained with mask. Ventilation were made with intralaryngeal mask airway blade (ILMA, fastrach). After remifentanyl 1 mg kg-1 and vecuronium 0.1 mg kg-1 were given, patient was entubated troogh "fastrach".Anesthesia was maintained with sevoflurane % 1 in O2 50 % and N2O 50 % mixture. Throughout the operation period, his all the vital signs were in normal ranges.After extubation the patient who has occurred upper airway collapse was entubated again and sent to intensive care unit. Fourth hour postoperatively patient was extubated and sent to ENT service on the 24th hour postoperatively.
