Özet
Hiponatremi, anestezistin sıklıkla karşılaşabileceği bir durumdur. Klinik belirtileri primer olarak nörolojiktir ve ciddiyeti genellikle ekstrasellüler hipoosmolalitenin gelişme hızı ile ilişkilidir. Genel anestezi verilecek hastalarda 130 mEq L-1 sodyum konsantrasyonu güvenli kabul edilir. Semptomlar olmasa bile, tüm elektif girişimlerde plazma sodyumu 130 mEq L-1 üzerine çıkarılmalıdır. Daha düşük konsantrasyonlar intraoperatif dönemde minimum alveolar konsantrasyonda azalma veya postoperatif dönemde ajitasyon, konfüzyon veya somnolansla kendini gösteren belirgin serebral ödeme neden olabilir. Hiponatremi tek başına ya da hipoksi ile beraber ciddi nörolojik defisite neden olabilir. Hiponatremi ve hipoksinin birlikte görüldüğü olgunun anestezi sonrası ortaya çıkan nörolojik defisiti tartışılmıştır.
Abstract
Hyponatremia is a frequent situation that many anaesthesists confront with. Clinical signs are primarely neurological and the severity is dependent on the rate of development of hyponatremia. Na concentration of 130 mEq L-1 is thought to be safe for patients who would undergo general anaesthesia. Although there is no clinical symptoms, plasma sodium should be raised above 130 mEq L-1 for all elective operations. Lesser concentrations cause peroperative decreased minimum alveolar concentration or postoperative significant serebral eudema causing agitation, confusion or somnolence. Hyponatremia by itself or in combination with hypoxia may cause severe neurological deficit. Postanaesthesic neurological deficit of a patient with hyponatremia and hypoxia was discussed in this paper.
