Türkiye'de anestezitlerin hava yolu idaresine ilişkin klinik uygulamaları, ekipman durumu eğitimi

Özet
Amaç: Günümüzde anestezistlerin hava yolu idaresinde kullanabileceği pek çok araç ve teknik mevcuttur. Bu çalışmanın amacı Türkiye'de hava yolu idaresine yönelik mevcut klinik uygulamaları, ekipman durumunu ve hava yolu konusundaki eğitimi değerlendirmektir.Gereç ve Yöntem: Bu çalışma 2004 Antalya Türk Anesteziyoloji ve Reanimasyon Kongresi’nde gerçekleştirilmiştir. Katılımcılardan hava yolu idaresine yönelik basit bir anket doldurmaları istendi.Bulgular: Kongreye katılan 1.057 katılımcıdan 312 kişi anket formunu doldurdu. Kıdemli asistanların sadece % 2'si uzmanların ise % 17.4'ü zor hava yolu konusunda bir kursa katılmıştı. Diğer katılımcılar hava yolu idaresindeki eğitimlerini sadece kendi eğitim hastanelerinde almış. Türkiye'deki anesteziyologların çoğu Amerikan Anestezistler Derneği zor hava yolu algoritmasını takip etmektedir. Anestezi indüksiyonu sırasında larengeal maske % 83-89 oranında katılımcıların ameliyathanelerinde mevcuttur. Katılımcıların % 65-73'ünün Fastrach kullanımı konusunda tecrübesi mevcuttur. Fiberoptik bronkoskop devlet hastanelerinin % 6'sında üniversite hastanelerinin ise % 68'sinde bulunmaktadır, ancak uzman hekimlerin sadece % 32.9'unun fiberoptik entübasyon deneyimi vardır.Sonuç: Türk anesteziyologların havayolu idaresine yönelik gelişmeleri takip ettikleri anlaşılmaktadır. Hava yolu idaresinde hem araştırma görevlilerini, hem de mezuniyet sonrasını kapsayacak ulusal bir eğitim programı, standartların daha da yükseltilmesine katkıda bulunabilir. Gelişme sağlanabilecek diğer bir saha ise, zor hava yolu olan hastalarda uyanık fiberoptik entübasyonun daha yaygın kullanılması olabilir.
Abstract
Aim: Nowadays a large number of airway devices and airway management techniques are available for the use of the anaesthesiologists. The aim of this study was to evaluate the current clinical practice, equipment availability and training about the airway management in Turkey.Materials and Methods: This study was conducted at the annual congress of Turkish Society of Anaesthesiology and Reanimation in Antalya, 2004. Participants were asked to complete a simple questionnaire about airway management.Results: Three hundred and twelve out of 1.057 participants of the congress have completed the questionnaire. Only 2 % of senior residents and 17.4 % of specialists have attended a training course about difficult airway management. The rest of the respondents had their training in airway management in their training hospitals. Most of the Turkish anaesthesiologists follow the American Society of Anesthesiologist’s difficult airway algorithm. Larengeal mask airway is available during anaesthesia induction in 83 % to 89 % of participants' operating rooms. Sixty five to seventy three percentages of respondents have experience with the use of Fastrach. The availability of fiberoptic scope ranges from 6 % in state hospitals to 68 % in university hospitals, whereas only 32.9 % of specialists had experience in fiberoptic intubation.Conclusion: Turkish anaesthesiologists seem to follow the current developments in airway management. A national training program for airway management for both residents and postgraduates would further help to improve the standards of care. Another potential area for improvement seems to be the widespread use of awake fiberoptic intubation in case of difficult airway.
