İntraoperatif sedasyonda propofol ve deksmedetomidinin hemodinami üzerine etki ve eşdeğer sedasyon skorlarında bispektral indeks değerlerinin karşılaştırılması

Özet
Amaç: Çalışmamızda spinal anestezi altında inguinal herni cerrahisi yapılan hastalara uygulanan intraoperatif sedasyonda, propofol ve deksmedetomidinin hemodinami üzerine etkilerini ve eşdeğer sedasyon skorlarında bispektral indeks (BİS) değerlerini karşılaştırmayı amaçladık.Gereç ve Yöntem: Elektif inguinal herni cerrahisi planlanan 40 hasta rasgele iki eşit gruba ayrıldı. Sedasyon indüksiyonu, Grup D'deki hastalarda 1 µg kg-1 deksmedetomidin iv (10 dk.'da) uygulanarak yapıldıktan sonra, 0.5-0.7 µg kg-1 sa-1 dozda deksmedetomidin infüzyonuna başlandı. Grup P'de sedasyona 1 mg kg-1 propofol iv (10 dk.'da) ile başlandı ve 3 mg kg-1 sa-1 dozda propofol infüzyonu ile devam edildi. İnfüzyon dozları sedasyon skalasına göre ayarlandı ve BİS değerleri kaydedildi. Operasyon sırasında ve sonrasında sedasyon düzeyini saptamak için 'Observer's Assessment of Alertness/Sedation' (OAA/S), intraoperatif ağrının değerlendirilmesinde Vizüel Analog Skala (VAS) kullanıldı. Bispektral indeks, ortalama arter basıncı (OAB), kalp atım hızı (KAH), periferik 02 saturasyonu (SpO2), solunum sayısı (SS) değerleri, OAA/S ve VAS skorları kaydedildi.Bulgular: Deksmedetomidin grubunda OAB ve KAH daha düşüktü ve bu istatistiki olarak anlamlı bulundu (p0.05). Abstract
Aim: In our study, we aimed to evaluate the effects of propofol and dexmedetomidine on hemodynamic and to compare bispectral index values between similar sedation scores during intraoperative sedation in patients undergoina inguinal hernia repair with spinal anesthesia.Materials and Methods: 40 patients undergoina elective inguinal hernia surgery were randomly allocated in to two groups. For sedation, dexmedetomidine in Group D and propofol in Group P injected iv in 15 minutes and later infusion of them sequentially applied. Infusion doses set in order to sedation scales and BIS values were recorded. Peroperatively or postoperatively “Observer's Assessment of Alertness/Sedation (OAA/S)”, 10 cm Visual Analog Scale pain score (VAS, 0=no pain, 10= severe pain) was used to determine the sedation level. Bispectral index, mean arterial pressure, heart rate, peripheral O2 saturation and breathe rate values and OAA/S and VAS scores were recorded.Results: In Group D the decreasin mean arterial pressure and heart rate were statistically significant. However, VAS scores during surgery were not. On the other hand, in Group P OAA/S and BIS scores were significantly lower at 5, 10 and 15 minutes just after beginning to sedation. In postoperative period there is no significantly difference in values of VAS, heart rate, mean arterial pressure, breathe rate and SpO2. At the same, postoperative OAA/S and BIS values were significantly low in Group D.Conclusion: The results oftlüs study demonstrats that sedation effect of dexmedetomidine begins and also finishes than propofol. More hipotensive and bradicardia effects are observed in the dexmedetomidine and in our opinion intraoparative sedation doses can adjust in respect of BIS values and it can be used unless increasing it's doses at high level.
