Özet

Tanısal amaçlı torakotomi yapılması planlanan spontan soluyan bir hastada rejyonel anestezi uyguladığımız bir olguyu sunmayı amaçladık. İntra torasik kitle tanısı için ameliyata alınan genel anesteziyi istemiyen 75 yaşında kronik obstruktif akciğer hastasında yüksek trokal epidural uygulandı. Hastaya T2-T3 aralığından 8 mL % 0.5 bupivakain ve 1 mL 50 Ìg fentanil ile epidural anestezi uygulandı ve kateter yerleştirildi. Torakotomi sonrası sağ akciğer spontan pnömotoraks ile kollabe oldu. Ameliyat spontan tek akciğer ventilasyonu ile başarılı şekilde sonlandırıldı. Yüksek torasik epidural anestezi ile torokotomi riskli hastalarda bir seçenektir.

Abstract

We report a case of diagnostic thoracotomy performed in a spontaneously breathing awake patient under regional anesthesia. High thoracic epidural anaesthesia was performed in a 75 years old patient with chronic obstructive pulmonary disease and refused to general anaesthesia undergoing thoracotomy for diagnosis of an intrathoracic mass. Epidural anaesthesia was performed at the T2-T3 level using 8 mL bupivacaine % 0.5 and 50 µg fentanyl. After thoracotomy the right lung was collapsed by spontaneous pnomothorax. The operation was accomplished successfully with spontaneous one lung ventilation. Thoracotomy with high thoracic epidural anaesthesia in awake patient is an option in certain high-risk patients.
