Postoperatif bulantı kusmanın önlenmesinde ondansetron-deksametazon kombinasyonunun ondansetron ve deksametazon ile karşılaştırılmas

Özet

Amaç: Bu çalışmada operasyon öncesi ondansetron-deksametazon kombinasyonunu veya bunlardan birini uygulayarak laparoskopik kolesistektomilerde görülen bulantı kusmanın ne ölçüde önlenebilineceğinin araştırılması amaçlandı.Gereç ve Yöntem: ASA I-III grubundan, yaşları 18-75 arasında olan 75 hasta 3 gruba ayrıldı. İndüksiyon öncesi; Grup-I'de 8 mg deksametazon, Grup-II'de 4 mg ondansetron, Grup-III'de 4 mg ondansetron + 8 mg deksametazon intravenöz olarak uygulandı. İndüksiyonda, iv, 2 mg kg-1 propofol, 0.5 µg kg-1 remifentanil bolus, 0.6 mg kg-1 rokuronyum uygulandı. % 50 oksijen-hava karışımı ve % 0.6-1 konsantrasyonda izofluran ve remifentanil infüzyonuyla idame yapıldı. Son 30. dakikada 75 mg diklofenak sodyum intramuskular uygulandı. Postoperatif 24 saat bulantı kusma ve ağrı takibi yapıldı. Bulantı ya da bulantı, kusma varlığında iv 10 mg metoklopramid, ağrı için 1 mg kg-1 iv tramadol uygulandı.Bulgular: Postoperatif 0-2. saatte ve 6-24. saatte bulantı, bulantı- kusma sıklığı Grup I'de diğer iki gruba göre daha fazlaydı (p< 0.05). Antiemetik ilaç kullanımı da 0-2. saatte, 6-24. saatlerde Grup I'de diğer iki gruba göre daha fazla gözlendi (p< 0.05). VAS değerleri ve ek analjezi gereksinimleri Grup I'de 2-6. saatler arasında diğer gruplara göre daha yüksekti. Diğer iki guruba göre tramadol ihtiyacı grup I de daha fazlaydı. (p< 0.05). Postoperatif baş ve yan ağrısı, abdominal distansiyon idrar retansiyonu, kaşıntı gibi komplikasyonlar Grup III'de en düşüktü.Sonuç: Laparoskopik kolesistektomilerde postoperatif bulantı kusmanın önlenmesinde ondansetron-deksametazon kombinasyonunun daha etkin olduğu bulundu.

Abstract

Aim: The aim of this study was to evaluate the effect of ondansetron and dexamethasone in combination or as single drug therapy for treatment of postoperative nausea and vomiting in patients undergoing laparoscopic cholecystectomy.Material and Methods: Seventy-five patients, ASA I-III, aged 18-75 years were randomly allocated into 3 groups. Before anesthetic induction Group I received 8 mg of dexamethasone, Group II received 4 mg of ondansetron and Group III received 4 mg of ondansetron and 8 mg of dexamethasone All drugs were parenterally. Anaesthesia was induced with 2 mg kg-1 propofol, 0.5 µg kg-1 remifentanil, and 0.6 mg kg-1 rocuronium and maintained with a remifentanil infusion. and isoflurane 0.6-1 % in 50 % air in oxygen. Thirty minutes before the end of surgery 75 mg of diclofenac sodium was administered intramuscularly. Postoperative nausea, vomiting and pain were evaluated. During the 24 hours following surgery. During the postoperative period metoclopramide (10 mg iv)for nausea and vomiting and tramadol (1 mg kg-1) were given for pain.Results: The incidence of nausea, nausea and vomiting were higher in Group I than the other two groups at postoperative 0-2 h and 6-24 h (p< 0.05). Antiemetic therapy was higher in Group I. at 0-2 and 6-24 hours (p< 0.05). VAS values and analgesic requirements were higher than other groups at 2-6 hours in Group I.Group I required significantly more tramadol for pain management postoperatifly compared to the other two groups (p< 0.05). The complications such as postoperative headache and side pain, abdominal distention, urinary retention, itching were lower in Group III.Conclusion: Ondansetron and dexamethasone in combination were effective the prevention of postoperative nausea and vomiting in patients undergoing laparoscopic cholecystectomy.
