ARDS'nin erken döneminde kortikosteroid tedavisi

Özet
Kortikosteroidler ARDS'nin tedavisinde değişik dönemlerde yıllardır kullanılmaktadır. Bulunduğu araçta bomba patlaması ile acil ünitesine getirilen hastada ciddi akciğer kontüzyonu saptanmıştır. Mekanik ventilasyona rağmen ciddi gaz değişimi sorunu olan hasta PEEP titrasyonu ve "recruitment manevrasına" yanıt vermemiştir. Hastaya ARDS'nin tedavisinde erken dönemde kortikosteroid tedavisi uygulanması sonucunda gaz değişiminde ve radyolojik görüntüde hızlı bir iyileşme kaydedilmiştir. Kortikosteroidlerin ARDS tedavisindeki önemi açısından bu olgu sunulmuştur.
Abstract
Corticosteroids have been tried in different phases of ARDS for several years. A 54 years female patient injured due to bomb explosion admitted to the hospital. She had severe oxygenation defect despite mechanical ventilation as a result of severe lung contusion. She did not respond to PEEP and recruitment maneuver. We decided to use corticosteroid at early phase of ARDS. After corticosteroid therapy gas exchange and radiologic findings improved rapidly. This case was presented because of the importance of corticosteroids in ARDS.
