Lomber disk cerrahisi yapılan hastalarda postoperatif analjezide lornoksikam ve tramadolün hasta kontrollu analjezi yöntemi ile etkinliklerinin karşılaştırılması

Özet
Amaç: Bu çalışmada postoperatif ağrı kontrolünde yaygın olarak kullanılan tramadol ile kullanıma yeni giren lornoksikamın intravenöz hasta kontrollü analjezi (HKA) yöntemi ile birlikte ve tek başına kullanıldığında etkinliğini karşılaştırmayı amaçladık.Gereç ve Yöntem: Genel anestezi altında ASA I-II risk grubundan, lomber disk cerrahisi planlanan, yaşları 21-60 arasında değişen toplam 81 hasta çalışmaya dahil edildi. Derlenme ünitesine alınan hastalar ağrı yakınması bildirdiklerinde rasgele olarak Grup 1 (bolus 1 mg kg-1 tramadol, tramadol infüzyon 10 mg saat-1), Grup 2 (lornoksikam 1 mg saat-1), Grup 3 (lornoksikam 1,5 mg saat-1) ve Grup 4 (tramadol 1 mg kg-1 bolus, lornoksikam 1 mg saat-1) olarak dört eşit gruba ayrıldı. Analjezik etkinliğin değerlendirilmesinde 10 cm'lik Vizüel Analog Skala (VAS), sedasyonun değerlendirilmesinde ise, Observer's Assessment of Alertness/Sedation skalası (OAA/S) kullanıldı. Ölçümler tedavinin başlangıcı, 30. dk., 1., 2., 3., 4., 8., 12., 16., ve 24. saatlerde kaydedildi.Bulgular: Grup 1 ve 4 karşılaştırıldığında VAS değerleri ilk 30. dk. 1.90±0.40 ve 1.60±0.30, 1. saat 1.40±0.40 ve 1.20±0.50, 2. saat 1.30±0.40 ve 1.00±0.30, 3. saat 1.30±0.40 ve 1.00±0.30 olarak grup 4'te daha düşük bulundu (p0.05). Abstract
Aim: In this study we aimed to compare the efficiency of tramadol, which is used widely in controlling postoperative pain, and lornoxicam, which enters usage newly, by using intravenous patient control anaesthesia method one by one and together.Materials and Methods: Under general anaesthesia, patient in status. ASA I-II, a total of 81 patients (21-60 years old), who planned to be performed lumbar disc surgery surgery were included the study. These patients in recovery unit are divided into four equal groups when they said pain complaint as group 1 (tramadol 10 mg/hour-1), group 2 (lornoxicam 1 mg/hour-1), group 3 (lornoxicam 1.5 mg/hour-1) and group 4 (lornoxicam 1 mg/hour-1 and 20 mg bolus tramadol). Visual analog scale (VAS), 10 cm., was used for efficiency of analgesia and Observer's Assessment of Alertness/Sedation scale (OAA/S) to evaluate the sedation. Measurements were obtained at the beginning of the treatment, at 30 minutes, first, second, third, fourth, eight, 12th,16th and 24th hour.Result: When group 1 and 4 compared the VAS values were obtained more lower at group 4 like at first 30 minutes 1.90±0.40;1.60±0.30, at first hour 1.40±0.40; 1.20±0.50, at second hour 1.30±0.40;1.00±0.30 and at third hour 1.30±0.40;1.00±0.30 (p0.05). The VAS values of group 1 and 4 were obtained lower then group 2 and 3 (p0.05).Conclusion: As a result, we concluded that, in postoperative analgesia, when we consider the efficiency and side effects, combination of tramadol and lornoxicam can provide maximum quality and patients pleasure.
