İki farklı zamanda uygulanan deksmedetomidinin ameliyat sonrası titreme üzerine etkisi

Özet
Amaç: Çalışmamızda desfluran ile genel anestezi uygulanan hastalarda ameliyatın başında veya sonunda deksmedetomidine infüzyonu uygulanmasının ameliyat sonrası titreme üzerindeki etkilerinin farklı olabileceği hipotezinin doğruluğunu araştırdık.Gereç ve Yöntem: Desfluran anestezisi altında total abdominal histerektomi ve bilateral salpingo-ooferektomi veya miyomektomi ameliyatı planlanan ASA I-II grubuna dahil 60 hasta rasgele üç gruba ayrıldı. Grup 1'de entübasyondan hemen sonra, grup 2'de ise, cerrahi bitiminde 1 µg kg-1 deksmedetomidin 5 dk. içinde uygulandı. Grup 3'teki hastalara deksmedetomidin uygulanmadı. Ameliyat sırasında nazofaringeal ısı değerleri, desfluran kesilmesi ile ekstübasyon arasında geçen süre, ameliyat sonrası bakım ünitesinde ise, ısı değerleri, titreme varlığı, Ramsay sedasyon skoru, modifiye Aldrete skoru, verbal ağrı skoru (VAS), analjezik tüketimi ve bulantı varlığı kaydedildi.Bulgular: Grup 1 ve grup 2'de titreme insidansı grup 3'e göre daha düşüktü (p< 0.001). Grup 2'de titreme insidansı grup 1'den düşüktü (p< 0.05). Ekstübasyon süresi grup 2'de diğer gruplara göre daha uzundu (p< 0.05). Ameliyat sonrasında 15., 30. ve 45. dk.'lardaki sedasyon skoru grup 2'de diğer gruplara göre daha düşüktü (p< 0.001). Ameliyat sonrası 60. dakikadaki VAS değeri grup 1 (p< 0.05) ve grup 2'de (p< 0.01) grup 3'e göre daha düşüktü. Ameliyat sonrası analjezik tüketimi grup 2'de grup 3'e göre daha düşüktü (p< 0.01). Bulantı insidansı grup 2'de grup 3'e göre düşüktü (p< 0.05).Sonuç: İndüksiyon sonrasında veya ameliyatın sonuna doğru deksmedetomidin uygulaması ameliyat sonrası titreme insidansını azaltmakla birlikte, geç deksmedetomidin infüzyonu erken uygulamaya kıyasla daha etkin olmuştur. Ayrıca, geç deksmedetomidin uygulaması daha iyi analjezi ve bulantı önleyici etki sağlamıştır, ancak bu uygulama ekstübasyon süresini uzatabileceği ve ameliyat sonrası ilk 45 dk. boyunca sedasyona neden olabileceği için hastaların yakın takibi gereklidir.
Abstract
Aim: We aimed to investigate the hypothesis that infusion of dexmedetomidine at the beginning or at the end of the operation could change the efficacy of the drug in preventing postoperative shivering.Material and Methods: 60 ASA I-II physical status patients undergoing total abdominal hysterectomy and bilateral salpingo-oopherectomy or myomectomy are randomly allocated into three groups. Dexmedetomidine 1 µg h-1 is infused in 5 minutes just after intubation (group 1) or at the end of the surgery (group 2). Patients in group 3 did no receive dexmedetomidine infusion. Nasopharyngeal temperature levels, the time from termination of desflurane application until extubation, the presence of shivering, Ramsay sedation score, modified Aldrete score, verbal pain score, the consumption of analgesics and the presence of nausea were recorded.Results: In group 1 and group 2, the incidence of shivering was lower than group 3 (p< 0.001). In group 2, shivering incidence was lower than group 1 (p< 0.05). In group 2, extubation time was longer than the other groups (p< 0.05). Sedation scores at 15, 30 and 45 minutes postoperatively were higher than the other groups (p< 0.001). In group 1 (p< 0.05) and group 2 (p< 0.01), verbal pain score at 60 minutes postoperatively was lower than group 3. The consumption of analgesics was lower in group 2 when compared with group 3 (p< 0.01). The incidence of nausea was lower in group 2 when compared with group 3 (p< 0.05).Conclusion: Although the administration of dexmedetomidine after intubation or at the end of surgery reduced the incidence of shivering, the late application was more effective when compared with early administration. In addition to better analgesia level, late administration of dexmedetomidine prevented nausea. But as this application can elongate extubation time and cause sedation during 45 minutes postoperatively, close observation of these patients is necessary.
