Özet

Amaç: Endoskopik cerrahi olgular›nda, batın içine CO2 insuflasyonu yapıldıktan sonra artan intraabdominal basınç, intratorasik basıncı da artırmakta, mide distansiyonu, biliyer ve gastroözefageal reflüye neden olabilmektedir. Artmış intratorasik basınç sonucu havayolu basıncı yükselebilmekte ve ventilasyon parametreleri olumsuz yönde değişebilmektedir. Bu çalışmada, endoskopik kolesistektomi anestezisinde endotrakeal entübasyon ile PLMA (Proseal LMA) uygulamasını, stres yanıt, hemodinamik etkiler, ventilasyon parametreleri ve gastroözefageal reflü açısından karşılaştırmayı amaçladık. Gereç ve Yöntem: Çalışmaya yaşları 20-75 arasında değişen (ASA I-II) laparoskopik kolesistektomi uygulanması planlanan 60 hasta dahil edildi. Kalp atım hızı (KAH), ortalama arter basıncı (OAB), periferik oksijen satürasyonu (SpO2), pik havayolu basıncı (Pmax) ve endtidal karbondioksit basıncı (ETCO2), mide ve nazofarenks pH’sı, plazma kortizol düzeyleri ölçülerek kaydedildi. Bulgular: OAB, ETT (endotrakeal entübasyon) grubunda entübasyondan 1 dk. sonraki ölçümde anlamlı düzeyde yüksek bulundu. PLMA grubunda kalp atım hızının entübasyondan sonraki değeri, preoperatif değerinden anlamlı olarak düşüktü. SpO2, ETCO2, Pmax, mide ve nazofarenks pH ölçümlerinde gruplar arasında farklılık yoktu. Gastroözofageal reflü düşündürecek nazofarenks pH düşüşü her iki grupta da kaydedilmedi. Grup PLMA’da entübasyondan 1 dk. sonraki kortizol değerleri preoperatif ölçümlerden anlamlı olarak düşük bulundu. Sonuç: Sonuç olarak, PLMA uygulamasının hemodinamik ve endokrin yanıtı olumlu yönde etkilediği, doğru yerleştirilmiş PLMA’nın etkin ventilasyon sağladığını gözlemledik. Gastroözefageal reflü açısından güvenilir olduğu kanaatine vardık. Endoskopik cerrahi olgularında PLMA’nın endotrakeal entübasyona alternatif olabileceğini söyleyebiliriz.

Abstract

Aim: In endoscopic surgery, the increase of intraabdominal pressure due to CO2 insufflations causes intrathorasic pressure to increase, distension of stomach, bilier and gastroesophageal reflux, increase of airway pressure and change of ventilation parameters. In this study, we aimed to compare PLMA implementation with endotracheal intubation in terms of stress response, hemodynamic effects, ventilation parameters and gastroesophageal reflux. Materials and Method: 60 patients with ages ranging between 20-75, included in the ASA I-II classification planned to undergo elective laparoscopic colesistectomy were included. Heart rate (HR), mean arterial pressure (MAP) and peripheral oxygen saturation (SpO2) peak airway pressure (Pmax), end tidal CO2 values, the gastric and nasopharyngeal pH, plasma cortisol levels were measured and recorded. Results: There were no significant differences between the two groups in terms of SpO2 values. MAP was found to be significantly higher in the ETT group following intubation. The HR following 1 minute after intubation was significantly lower when compared with the preoperative value in the PLMA group. There were no significant differences between the two groups in all stage measurements in term of ETCO2 and Pmax values. There were no significant differences between the groups in terms of gastric and nasopharyngeal pH measurements. Nasopharyngeal pH fall due to gastroesophageal reflux was not recorded. In group PLMA, cortisol values were found to be significantly lower 1 minute after intubation when compared with preoperative measurements. Conclusion: As conclusion, we observed that the PLMA implementation effected both hemodynamic and endocrine response positively and that a correctly inserted PLMA provided effective ventilation. We conclude that it was reliable in terms of gastroesophageal reflux. We can propose that it can be an alternative for the endotracheal intubation in endoscopic surgical interventions.
