Özet

Postoperatif analjezi; cerrahi işlem uygulanan hastalarda göz önünde bulundurulması gereken önemli bir sorundur ve bu amaçla opioidler, NSAIİ'lar, lokal anestezikler ve bunların kombinasyonu kullanılmaktadır. Hasta Kontrollü Analjezi (HKA) temelli analjezik tüketimi, analjezi derecesini saptamada çok kesin olmayan, ama ilaç gereksinimini objektif olarak belirleyebileceğimiz mevcut tek metoddur. Bu çalışmada, laminektomi operasyonu planlanan olgulara preoperatif intravenöz lornoksikam uygulanmasının, HKA'de tramadol gereksinimi üzerine etkilerinin karşılaştırılması amaçlandı. Laminektomi operasyonu geçirecek ASA I-II, 36 olgu K ve L olmak üzere rasgele iki gruba ayrıldı. Operasyondan 30 dk. önce intravenöz yoldan K grubu olgulara 10 mL SF ve T grubu olgulara ise 8 mg (10 mL) lornoksikam uygulandı. Standart anestezi indüksiyonu ve idamesi gerçekleştirilen olguların; operasyon bitimini takiben kas gevşetici ve narkotik analjezik ilaç etkileri antagonize edildi. Derlenme odasına alınan olgulara; intravenöz HKA uygulanarak bazal+PCA modunda tramadol uygulanmaya başlandı. Olgular arasında; demografik veriler, operasyon süresi ve anestezi süreleri açısından anlamlı bir fark olmadığı (p>0.05) saptandı. Abstract

Post-operative analgesia is an important situation in patients undergoing surgical intervention and should be taken into account. Opioids, NSAIDs, local anesthetics or combination of these are used for these purpose. Analgesic consumption, based in Patient Controlled Analgesia (PCA) is the only method to evaluate the drug requirement objectively although not precisely determining the analgesia level. The purpose of this study is to compare the effect of pre-operative i.v. larnoxicam application to post-operative PCA tramadol consumption in patients undergoing laminectomy operation. Thirty-six, ASA I-II patients undergoing laminectomy operations were enrolled in this study and allocated into two groups (Group K and Group L) randomly.. In group K cases were treated with 10 mL saline and in group L with 8 mg (10 mL) lornoxicam, 30 minutes prior to surgery. After a standardized induction of anesthesia and maintanence; muscle relaxants and narcotics were antagonized at the end of the operation. Intravenous PCA device were applied to the patients in the recovery room and treated with tramadol in basal + PCA mode. There were no significant differences for the demographic properties, operation and anesthesia times between the cases (p>0.05).
