Özet
Giriş: Propofol anestezi indüksiyon ve idamesinde kullanılan sedatif hipnotik bir ajandır. Kardiyovasküler depresyon yapabildiğinden, indüksiyon sırasında yavaş infüzyonu önerilmektedir. Çalışmamızda, propofolü infüzyon pompasıyla farklı hızlarda vererek indüksiyon için uygun propofol infüzyon hızını saptamayı amaçladık. Gereç ve Yöntem: Bu prospektif randomize çalışma ASA I-II sınıfı 18-65 yaş arası 100 hastada yapıldı. Tüm hastalara midazolam ile premedikasyon uygulandı. Standart monitörizasyon sonrası 1.5 mcg kg-1 dozunda iv fentanil verildi. Hastalar rasgele seçim ile yirmişer kişilik 5 gruba ayrılıp, 200-400-600-800-1000 mL sa-1 propofol infüzyonu kirpik refleksi kaybolana kadar uygulandı. Verilen propofol miktarıyla indüksiyon süresi kaydedildi. Arter kan basıncı ve kalp hızı indüksiyondan önce ve sonra beş kez her dakika kaydedildi.Bulgular: Kirpik refleksinin kaybolması için gereken doz Grup I’den V’e sırasıyla 0.81±0.26, 1.41±0.7, 1.68±0.67, 2.35±0.82 ve 2.27±0.62 mg kg-1 vücut ağırlığı olup, gruplar arasında anlamlı olarak farklıydı (p
Abstract
Objectives: Propofol is a hypnotic sedative drug which is used for anaesthesia induction. To avoid cardiovascular depression of propofol, slow infusion is recommended during induction. The aim of this study is determination of best propofol dose with different infusion rates of propofol administered by syringe pumps. Material and Methods: This prospective, randomized study was performed on 100 patients of ASA I or II aged 18-65 years. All patients were premedicated with midazolam. Standard monitoring was followed by fentanyl 1.5 mcg kg-1 iv before the patients were randomly allocated to one of five groups each including 20 patients. Groups I, II, III, IV and V were infused propofol in rates of 200, 400, 600, 800 or 1000 mL h-1, consecutively until loss of eyelash reflex. The duration of induction was recorded with the amount of propofol infused. The induction dose in mg kg-1 body weight was calculated. Arterial blood pressure and heart rate were recorded before and five consecutive minutes following induction.Results: The doses required to abolish eyelash reflex was 0.81±0.26, 1.41±0.7, 1.68±0.67, 2.35±0.82 and 2.27±0.62 mg kg-1 body weight for Groups I to V consecutively and found significantly different between groups (p
