Sezaryan sonrası epidural analjezide sürekli levonbupivakain infüzyonuna üç farklı dozda fentanil ilavesi

Özet
Amaç: Bu çalışmada, epidural anesteziyle elektif sezaryen ameliyatı sonrası, % 0.125 levobupivakain ile sürekli epidural infüzyona ilave edilecek üç farklı dozda fentanilin; postoperatif ağrı, yan etkiler ve hasta memnuniyeti üzerine olan etkisi araştırılmıştır. Gereç ve Yöntem: Elektif sezaryen operasyonu planlanan ASA I-II grubundan, 18-45 yaş arası, 60 gebe çalışmaya dahil edildi. % 0.125 levobupivakain infüzyonuna Grup I-II-III ve IV’de sırası ile 0-1-2-3 mcg ml-1 fentanil ilave edildi. Gebeler, postoperatif 0,1/2,1,2,3,4,6, 8,12,24. saatlerde ağrı, motor blok, sensoriyel blok ve yan etkiler açısından (bulantı, kusma, kaşıntı, sedasyon, hipotansiyon, bradikardi, solunum depresyonu-idrar retansiyonu) değerlendirildi. Bulgular: Genel olarak III ve IV. Gruptaki hastaların istirahatte ve harekette VAS skorları, I ve II. Gruptakilere göre daha düşüktü (p0.05). Sonuç: Sonuç olarak, sezaryen sonrası epidural analjezide, % 0.125 konsantrasyonda sürekli levobupivakain infüzyonuna ilave edilen 2 mcg ml-1 fentanil dozunun, yan etki sıklığını arttırmaksızın, ağrı şiddeti ile birlikte, ilave analjezik gereksinimini azaltarak, hasta memnuniyetini arttıran, optimal doz olduğu kanaatine varıldı.
Abstract
Introduction: In this study, effect of continuous 0.125 % levobupivacaine epidural infusion with fentanyl added in three different doses on postoperative pain, side effects and patient’s satisfaction scores were investigated after elective cesarean operation under epidural anesthesia. Material and Methods: Sixty pregnants, ages between 18-45 years, from ASA I-II groups, were included in the study. 0.125 % levobupivacaine solution was used in all groups and in Groups II,III and IV; 1,2 and 3 mcg ml-1 fentanyl were added respectively. All pregnants were evaluated in terms of pain, motor block, sensory block and side effects at postoperative 0,1/2,1,2,3,4,6,8,12,24 hours. VAS scores at rest and at movement were lower in Groups III and IV than Groups I and II (p0.05). Conclusion: As a result, it was concluded that 2 mcg ml-1 fentanyl added to the continuous 0.125 % levobupivacaine infusion for epidural analgesia after cesarean operation was the optimal dose which decreases the additional analgesic requirement and the intensity of pain, and increases patient’s satisfaction without increasing side effect frequency.
