Özet

Giriş: Çalışmada, jinekolojik alt batın cerrahisinde epidural ve genel anestezi uygulaması kombinasyonu ile standart genel anestezi uygulamasının; intraoperatif cerrahi stres yanıt ve postoperatif analjezik tüketimi ile gastrointestinal fonksiyonlar üzerine etkilerinin karşılaştırılması amaçlandı. Gereç ve Yöntem: Abdominal histerektomi uygulanacak ASA I-II grubundan 50 olgu rasgele iki gruba ayrıldı. Olgulara; epidural+genel anestezi uygulama grubunda, ameliyattan önce lomber epidural kateter yerleştirilerek 10 mL % 0.75 ropivakain verildikten sonra genel anestezi, genel anestezi grubunda standart genel anestezi uygulandı. Non-invaziv kan basıncı değerleri ve kalp atım hızı, anesteziden önce ve ameliyat sonrası 30. dakikaya kadar 10 dk. aralıklarla kaydedildi. Kan şekeri, kortizol, ACTH, insülin değerleri ameliyattan önce (kontrol) ve ameliyat sırasında (intraoperatif) stres yanıt kriterleri olarak ölçüldü. Ameliyat sonrası ağrı tedavisi intravenöz hasta kontrollü analjezi yöntemiyle morfin kullanılarak sağlandı. Post-operatif 24 saatlik toplam analjezik tüketimi, gaz-gaita çıkış zamanları ile yan etkiler kaydedildi.Bulgular: Kan şekeri, kortizol ve ACTH’nın intraoperatif değerlerinin her iki grupta kontrol değerlerle karşılaştırıldığında arttığı saptandı (p

Abstract

Introduction: In our study we aimed to compare the effects of combined epidural-general anaesthesia on intraoperative stress response, postoperative analgesic consumption and recovery of gastrointestinal function with standard general anaesthesia in total abdominal hysterectomy. Materials and Methods: 50 patients in ASA I-II physical status undergoing elective abdominal hysterectomy were randomly assigned to two groups. In epidural+general anaesthesia group lomber epidural catheter was placed and general anaesthesia was used following 10 ml of 0.75 % ropivacaine administration through the catheter. Standard general anaesthesia was applied to the patients in general anaesthesia group. Non invasive blood pressure and heart rate values were recorded before anaesthesia and at 10 min intervals until postoperative 30 min. Serum values of glucose, cortisol, ACTH and insulin were measured before operation (control) and intraoperatively as stress response parameters. Postoperative pain management was obtained with intravenous patient controlled analgesia by using morphine. Total analgesic consumption for postoperative 24 hours, time to first flatus and defecation and adverse effects were recorded.Results: Serum glucose, cortisol and ACTH values were higher intraoperatively compared to control values in both groups (p
