Omuz cerrahisinde postoperatif analjezi sağlama yöntemlerinin karşılaştırılması

Özet
Amaç: Omuz cerrahisi uygulanan hastalarda postoperatif ağrı önemli bir sorundur. Çalışmamızda interskalen kateterle (İSK), tek doz İS blok sonrası takılan subakromiyal kateterin (SAK) postoperatif analjezik etkinliklerinin karşılaştırılması amaçlandı.Gereç ve Yöntem: Etik kurul ve hasta onayı sonrası ASA I-II 36 hasta randomize olarak ikiye ayrıldı. İki gruba da preoperatif dönemde 30 mL % 0.5 bupivakainle İSB yapıldı. Kateterler Grup I’deki hastalara İSK ve Grup II’deki hastalara SAK olarak yerleştirildi. Postoperatif dönemde kateterlerden hasta kontrollü analjezi (HKA) uygulanmaya başlandı, HKA’da % 0.125 bupivakain GI’de 5 veG II’de 2.5 mL bazal infüzyon; GI’de 5 mL ve GII’de 2.5 mL bolus doz olarak verildi; kilitli kalma süresi 20 dakikaydı. Hastaların postoperatif vizuel analog skala (VAS) değerleri (0-10 cm) 0, 4, 8, 12 ve 24. saatlerde sorgulandı, ek analjezik ihtiyaçları kaydedildi. 24 saat sonunda hasta memnuniyeti (HM) sorgulandı.Bulgular: Her iki grupta da tüm hastalarda yeterli analjezi sağlandı (VAS< 4). Grup I’in VAS değerleri tüm zamanlarda anlamlı olarak daha düşüktü; sadece postoperatif 0. ve 4. saatlerde iki grup arasında fark yoktu. GI’de 2 hastanın (% 11) ve GII’de 8 hastanın (% 44) ek analjezik ihtiyacı oldu. Ek analjezik ihtiyacı açısından GI ile GII arasında fark yoktu. Abstract
Aim: Postoperative pain is a problem after shoulder surgery. The aim was to compare the postoperative analgesic efficacies of subacromial infusion after single shot interscalene block (ISB) to a continuous ISB.Material and Methods: After ethical committee approval and informed consent 36 ASA I-II patients were randomly divided into two groups. Preoperatively all patients received an ISB with 30 mL 0.5 % bupivacaine. Catheters were inserted to the interscalene sheath in Group I and to the subacromial space in Group II. In the postoperative period patients received an analgesia using patient controlled analgesia (PCA) device. The PCA parameters were 5 mL h-1 bupivacaine 0.125 %, a bolus of 5 mL bupivacaine 0.125 % in GI and 2.5 mL h-1 bupivacaine 0.125 %, a bolus of 2.5 mL bupivacaine 0.125 % in GII a with a lock out time of 20 minutes. Pain relief was assessed using a visual analog scale (VAS, 0-10 cm) at the postoperative at 0, 4th, 8th, 12th and 24th hours. Additional analgesics were noted and at the end of 24h patients rated their satisfaction.Results: All patients had adequate analgesia (VAS< 4). GI had lower VAS scores during the study period accept for the 0 and 4th hour compared to GII.
