Evde enteral beslenme uygulanan olgulardaki deneyimlerimiz

Özet
Amaç: Enteral beslenme gastrointestinal sistemi sağlam, ancak oral alımı yeterli olmayan hastalarda uygulanan bir yöntemdir. Çalışmamızda, evde enteral beslenme uygulanan olgulardaki deneyimimizi sunmayı amaçladık. Gereç ve Yöntem: Etik kurul onayından sonra, 1 Ocak 2004-31 Ağustos 2006 tarihleri arasında evde enteral beslenme uygulanarak takip edilen erişkin olguların kayıtları retrospektif incelendi. Bulgular: Çalışma süresinde 54 olgunun kayıtları değerlendirildi. Olgular 57.0±15.8 (ortalama±standart deviasyon) yaşında olup, 31 olgu (% 57.4) erkek, 23 olgu (% 42.6) kadın idi. Olguların % 59.3’ünün onkolojik, % 31.5’inin nörolojik bir hastalığı mevcuttu. Evde enteral beslenme % 36.8 cerrahi jejunostomi, % 28.1 nazogastrik tüp ile uygulanmıştı. Otuzaltı (% 66.7) olguda ürünler pompa ile uygulanmış, 22 (% 40.7) olguda gündüz aralıklı uygulama tercih edilmişti. Olguların takibi döneminde 12 olguda (% 22.2) kabızlık ve 5 olguda (% 9.3) bulantı olduğu saptandı. Aspirasyon ve kateter tıkanıklığına ise hiçbir olguda rastlanmadı. Olguların toplam evde beslenme süresi 8962 gün idi. Evde beslenme yerine hastanede yatarak beslenmeleri halinde oluşacak maliyet 106.199.7 YTL olarak hesaplandı. Bu dönemde 34 olgu (% 63.0) yaşamakta iken, 20 olgu (% 37.0) kaybedilmişti. Sonuç: Evde enteral beslenmenin, oral yoldan beslenmesi yetersiz olan hastalarda beslenme desteği için güvenle uygulanabildiği ve maliyeti azalttığı saptandı
Abstract
Aim: Enteral nutrition is a method for patients who cannot maintain oral intake with a functioning gastrointestinal tract. The aim of our study was to report our experience in patients with home enteral nutrition. Materials and Methods: After Ethics Committee approval, we retrospectively analyzed the data from all adult patients with home enteral nutrition which were followed by Nutrition Department from 01 January, 2004 to 31 August, 2006. Results: During the study period, it was evaluated the data of 54 patients. The mean age of all patients was 57.0±15.8 (mean±standard deviation) years, and 31 of them (57.4 %) were male, 23 (42.6 %) were female. The distribution of patients by disease was; (59.3 %) had oncologic, and (31.5 %) neurological disorders. Home enteral nutrition was administered via surgical jejunostomi in 36.8 % patients, via a nasogastric tube in 28.1 % patients. A mechanical pump was used in 36 (66.7 %) of the patients, and the mode of administration of home enteral nutrition was cyclical by daytime for 22 patients (40.7 %). During the course of home enteral nutrition, 12 patients (22.2 %) had constipation and 5 patients (9.3 %) had nausea. No aspiration and leakage in tube were detected. The total duration of home enteral nutrition was 8962 days. If the patients were hospitalized, than the additional cost were calculated as 106.199.7 YTL. During the study period, 34 patients (% 63.0) were alive, 20 (% 37.0) had died. Conclusion: Home enteral nutrition is a safe technique for nutrition support, providing nutrients to patients not able to eat orally, and it is cost-effective approach.
