Özet

Kalıcı trakeostomili bir hastada servikal pleksus bloğu ile karotid endarterektomi (KEA) ameliyatı sunulmuştur. Bu olgu, trakeostomi varlığı nedeni ile söz konusu olan, anatomik işaret noktaların bulunması, hava yolunun korunması ile oksijen desteğinin sağlanması ve etkili klinik nörolojik değerlendirme yapılması için iletişim kurulabilmesi gibi olası güçlükler bakımından ele alınmıştır. Servikal blok uygulaması başarı ile gerçekleştirildi, hasta sakin ve koopere idi, ameliyat sorunsuz şekilde tamamlandı.

Abstract

We describe a patient with a longstanding tracheostomy who underwent carotid endarterectomy (CEA) after a cervical plexus block had been administered. In this individual, the challenges of anesthetic management, which were caused by his tracheostomy, included difficulty in differentiating the anatomic landmarks used to direct the placement of the regional blockade, the administration of supplemental oxygen, and ensuring effective clinical neurologic monitoring. The cervical plexus block was effective, the patient was calm and cooperative during the procedure, intraoperative course was uneventful
