Özet

Giriş: Peripartum kardiyomiyopati; gebeliğin son bir ayı ile postpartum ilk beş ay arasında görülebilen, nedeni bilinmeyen ve insidansı canlı doğumlarda 1/1.300 ile 1/15.000 arasında değişen ciddi bir ventriküler yetmezlik tablosudur. Göğüs ağrısı, dispne, ortopne ve pretibiyal ödem ile seyreder. Akciğer grafisinde kardiyomegali veya plevral effüzyon, elektrokardiyografide (EKG) de düzensiz atımlar gözlenebilir. Tanı, ekokardiyografide (EKO) pompa fonksiyonunda bozulmanın veya ejeksiyon fraksiyonunda (EF) azalmanın saptanmasıyla konur. Bu makalede, peripartum kardiyomiyopati gelişen ve acil sezaryen planlanan gebedeki sedasyon ve epidural anestezi uygulaması deneyimimizi sunuyoruz.Olgu: Otuz dört yaşında hasta, nefes darlığı ve bacaklarda şişlik yakınmaları ile dış merkezde gebeliğinin 253. haftasında gestasyonel hipertansiyon (HT) tanısı almış. Ortopne gelişen ve kan basıncı 180/120 mmHg olan hastanın EKG’sinde sol dal bloğu ile birlikte anteroseptal ve inferiyor miyokard enfarktüsü saptanmış. Akciğer grafisinde de sağ bazalde plevral effüzyon gelişen hasta üniversitemize 255 haftalık gebe iken sevk edildi. EKO’sunda; EF % 40 ve sol atriyum ile ventrikül boyutu büyük olarak saptandı. Dilate kardiyomiyopati tanısı alan hastaya medikal tedavi planlandı ve acil doğum önerildi. Olguya ameliyathanede standart monitorizasyondan sonra 2 mg midazolam iv yapıldı, maske ile 6 L dk-1 O2 verildi. Sol radiyal arter ve sağ internal juguler ven kateterizasyonu uygulandıktan sonra L4-5 intervertebral aralık seviyesinden epidural kateter yerleştirildi. Kırk mg % 2’lik lidokain ile test yapıldı ve 25 mg % 0.25’lik bupivakain ile yeterli anestezi düzeyi (T4) sağlandı. Vital bulgular stabil seyretti. Cerrahi işlem 60 dakikada komplikasyonsuz tamamlandı. Postoperatif dönemde semptomları gerileyen olgu, 15. günde medikal tedavi ile taburcu edildi.Sonuç: Epidural anestezi, peripartum dilate kardiyomiyopati tanısı alan ve sezaryen operasyonu planlanan hastalarda kullanılabilecek güvenilir bir yöntemdir.

Abstract

Introduction: Peripartum cardiomyopathy (PPCM) is a rare but serious disorder in which initial left ventricular systolic dysfunction and symptoms of heart failure occur between the late stages of pregnancy and the early postpartum period of unknown etiology. The incidence of the disease varies between 1:1.300 and 1:15.000. Most common symptoms are chest pain, dyspnea, orthopnea, and peripheral edema. Recently, inclusion of echocardiography has made diagnosis of PPCM easier and more accurate. We summarized a case of peripartum dilated cardiomyopathy presenting for caesarean section, which was successfully managed with epidural anesthesia.Case Report: A 34 years old parturient (253 weeks) who diagnosed as gestational hypertension as the blood pressure was 180/120 mmHg, and also presenting dyspnea, orthopnea, and peripheral edema was transferred to our hospital. At the arrival to our hospital, further examination and tests were applied to the patient; there was left bundle branch block, anteroseptal and inferior myocardial infarction on ECG, pleural effusion on the right lung was seen on chest x-ray. Echocardiography had shown dilated cardiomyopathy, with an ejection fraction of 40 %, a preponderance of right heart findings, and pulmonary hypertension. Epidural anesthesia was performed at L4-5 interspace. 40 mg of 2 % lidocaine and 25 mg of 0.25 % bupivacaine were injected into the epidural space. The patient's haemodynamic status was monitored with IBP at radial artery, ECG, pulse oximetry and central venous pressure. The block was adequate at T4 level and vital signs were stable. Patient's perioperative course was lasted one hour and was uneventful. She was discharged on 15th day with medical therapy.Conclusion: In patients suffering from peripartum cardiomyopathy, undergoing caesarean section epidural anesthesia with invasive monitoring is an acceptable anesthetic alternative.
