Sol ve sağ torasik sempatektominin peroperatif hemodinamiye etkilerinin karşılaştırılması

Özet
Amaç: T2-T3 endoskopik torasik sempatektomi; palmar hiperhidrozis için etkin ve güvenli bir tedavi yöntemi olarak uygulanmaktadır. Sol T2 ve T3 gangliyonlar kalbin doğrudan innervasyonundan sorumlu olup, vazomotor sinirlerin parçalarıdır. Bu nedenle, çalışmamızda sağ ve sol torasik sempatektominin peroperatif dönemde hemodinamik ve kardiyak fonksiyonlar üzerine etkilerinin karşılaştırılmasını amaçladık. Gereç ve Yöntem: Çalışmaya ASA-I grubundan yaşları 21 ile 29 arasında değişen 40 erkek olgu alındı. Olgular rasgele 2 gruba ayrıldı. Grup I (n=20) olgulara sol T2-T3, Grup II (n=20) olgulara sağ T2-T3 torasik sempatektomi uygulandı. Tüm olgulara standart genel anestezi yöntemi uygulandı. Operasyon boyunca, sempatektomi öncesi ve sonrasında; kalp atım hızı, kan basıncı, non-invaziv kardiyak output cihazı ile kardiyak output, atım volumü ve kardiyak indeks ölçüldü. Ayrıca, sempatektomiden önce ve sonra bilateral palmar sıcaklık değerleri kaydedildi.Bulgular: Çalışmamızda yalnızca sol sempatektomiden sonra, sempatektomi öncesine göre; sistolik, diyastolik ve ortalama arter basınçlarında istatistiksel olarak anlamlı bir azalma saptandı (p
Abstract
Aim: Endoscopic thoracic sympathectomy at T2-T3 is an effective and safe treatment for palmar hyperhidrosis. Left T2 and T3 ganglia are in the direct pathway of sympathetic innervation of the heart and part of the vasomotor nerves. So, we investigated and compared the possible hemodynamic and cardiac functions changes during left and right thoracoscopic sympathectomies.Material and Methods: ASA I, 40 male patients between 21 and 29 ages were included in the study and they were divided randomly into two groups. Patients in group I (n=20) went under left T2-T3 thoracic sympathectomy and group II (n=20) went under right T2-T3 thoracic sympathectomy. All patients were given standard general anesthesia. Heart rate, non-invasive blood pressure, cardiac output, stroke volume and cardiac index by NICO (Non-invasive cardiac output) and bilateral palmar temperature were monitorized and recorded through out the operation, before and after the thoracic sympathectomy.
