Management of anesthesia in left sleeve pneumonectomy through bilateral thoracotomy

Özet

Bu makalede, sol ana bronş karsinomu tanısı ve karina invazyonu olan bir olgu sunuldu. Bu hastaya mediyastinoskopi ve aynı seansta bilateral ardışık torakotomi yoluyla sol “sleeve” pnömonektomi yapıldı. Anestezi uygulamasının sürdürülmesinde, torakal epidural analjezi ile kombine edilmiş genel anestezi kullanıldı. Karina rezeksiyonu ve trakea ile sağ ana bronşun uç uca anastomozu sırasında oksijenasyonu sağlamak için yüksek frekanslı jet ventilasyon uygulandı. Sonuç olarak, yüksek frekanslı jet ventilasyon ile yeterli oksijenasyon korunup, iyi cerrahi görüş alanı sağlandı. Operasyonlar sırasında ve sonrasında torakal epidural analjezi etkindi.

Abstract

A case diagnosed as the carcinoma of the left main bronchus with carinal invasion was presented in this article. The male patient underwent mediastinoscopy and left sleeve pneumonectomy through bilateral sequential thoracotomy at the same session. For the maintenance of anesthesia, thoracic epidural analgesia in combination with general anesthesia was used. High-frequency jet ventilation was applied for oxygenation during carina resection and end- to- end anastomosis of the trachea to right main bronchus. As a conclusion, adequate oxygenation was maintained and good surgical exposure was provided with high-frequency jet ventilation. Thoracic epidural analgesia was effective both during and immediately after operations.
