Özet

Amaç: Bu çalışmada amacımız ileri yaşta ürolojik cerrahi geçiren hastalarda anestezi uygulama yöntemi, anesteziyönetimi, postoperatif komplikasyon, kısa dönem mortalite ve morbiditenin araştırılmasıdır.Gereç ve Yöntem: Geriye dönük planlanan çalışmada hastane etik komite izni sonrası 2008-2009 yıllarında ürolojikliniğinde operasyon geçirmiş olan 65-79 yaş arası 50 olgu ile 80 yaş ve üstü 50 olgu 2 grup olarak çalışmaya alındı.Hastaların dosyalarından anestezi-cerrahi bilgileri, yoğun bakım izlem formları, hastane çıkış epikrizleri incelenerekelde edilen veriler değerlendirildi. Bireysel özellikler, ASA skorlaması, yandaş hastalıklar, sigara kullanımı, geçirilmişoperasyon varlığı, intraoperatif olarak anestezi uygulama yöntemleri, operasyon tipi, kullanılan anestezik ilaçlar vepostoperatif analjezikler kaydedildi. Yoğun bakım bilgileri, yoğun bakım-hastane kalış süreleri, gelişen komplikasyonlarve taburculuk şekilleri belirlendi. Bulgular: Cinsiyet, ASA skorlaması, preoperatif yandaş hastalıklar ve sigara kullanım öyküsü açısından iki grup ara- sında fark bulunmadı. Anestezi uygulama yöntemi olarak 65-79 yaş grubunda 13 hastada (% 26) spinal anestezi, 24hastada (% 48) genel anestezi, 6 hastada (% 12) epidural anestezi ve 7 hastada (% 14) sedasyon; 80 yaş ve üstü gruptada 9 hastada (% 18) spinal anestezi, 28 hastada (% 56) genel anestezi, 3 hastada (% 6) epidural anestezi ve 10 has- tada (% 20) sedasyon seçildiği saptandı İndüksiyon ajanı olarak 65-79 yaş grubunda tiyopental kullanımının propofolve etomidata göre daha fazla olduğu görüldü (p=0.032). Abstract

Aim: We aimed to evaluate retrospectively anesthetic technique, anesthetic management, postoperativecomplication(s), short term mortality and morbidity in old patients who had urologic surgery. Material and Method: After obtaining approval of the Ethics Commitee, patients who had undergone surgery inurology clinics between 2008-2009 were grouped in two categories as 50 patients aged 65-79 years in Group I, and≥ 80 years in Group 2. Data about anesthetic and surgical management,ICU montorization, and patients’ medical infor- mation were recruited from hospital fles for further evaluation. Demographic characteristics, ASA scores, details aboutco-existing diseases, smoking habits, previous surgical management , intraoperative anesthetic technique, intraopera- tive and postoperative anesthetic agents used were recorded. ICU records, ICU and hospital stay times, complicationsand patients’ health status at discharge were determined. Results: There was no difference between two groups in terms of gender, ASA scores, concomitant diseases andsmoking habbit. In Group 1; spinal anesthesia was used for 13 patients (26 %), general anesthesia for 24 patients (48%), epidural anesthesia for 6 patients (12 %) and sedation was performed in 7 patients (14 %). In Group 2, 9 patientshad received spinal (18 %), 28 patients general (56 %), 3 patients epidural anesthesia (6 %) and 10 patients weresedated (10 %). In 65-79 age group , as an induction agent thiopental was used more frequently than propofol andetomidate (p=0.032). As a difference between groups, in the treatment of postoperative pain, combination drug the- rapy was favoured more than the other medications per se in the comparatively older Group 2. (p
