Sigara içiminin preoperatif anksiyete üzerine etkisi

Özet
Amaç: Sigara içiminin kronik olarak anksiyete yaratıcı etkisinin olduğu gösterilmiştir. Çalışmamızın amacı kronik olarak sigara içen ve içmeyen hastaların preoperatif dönemde ölçülen anksiyete skorları arasında fark olup olmadığını araştırmaktır. Gereç ve Yöntem: Elektif cerrahi operasyon geçirecek 18-65 yaş arası, ASA I-II, toplam 300 hasta çalışmaya alındı. En az 2 yıl boyunca, günde en az 10 adet ve üzerinde sigara içen hastalar Grup S (n=150), hiç içmemiş olanlar Grup K (n=150)’ya alındı. Hastalara premedikasyon uygulanmadı. Hastaların bireysel verileri, hastalara uygulanacak olan anestezi türü, geçirilmiş olan operasyon öyküsü, sistemik hastalık öyküsü ve öğrenim durumları kaydedildi. Grup S’de hastaların günde kaç adet sigara içtikleri, kaç yıldır içtikleri ve en son kaç saat önce içtikleri (yoksunluk süresi) kaydedildi. Hastaların durumluk ve sürekli kaygı ölçekleri (DKÖ-SKÖ) State-Trait Anxiety Inventory (STAI) ile değerlendirildi. STAI formunun tamamlanmasından sonra, hastalardan stres düzeylerini numerik stres skora (NSS) göre belirtmeleri istendi. Bulgular: Hastaların kişisel özellikleri incelendiğinde gruplar arasında istatistiksel olarak anlamlı farklılık saptanmadı. Abstract
Aim: It has been shown that smoking is chronically anxiogenic. The aim of the study is to investigate the effect of smoking on preoperative anxiety in patients undergoing elective surgery. Material and Method: Three hundred patients, aged 18-65 years, ASA I-II who was scheduled for elective surgery were enrolled in the study. The patients smoked 10 or more cigarettes per day for at least 2 years were enrolled in Group S (n=150) while non-smokers were included in Group K (n=150). No premedication was given to the patients. Demographic data, type of operation and anesthesia, history of previous surgery, systemic disease and status of education of the patients were noted. Cigarettes per day, years smoking and abstinence time were noted in Group S. The patients’ state and trait anxiety scores (STAI-S, STAI-T) were measured by using State-Trait Anxiety Inventory (STAI). After the completion of STAI form, patients were requested to asses their stress levels according to the numerical stress score (NRS). Results: There was no difference between two groups for demographic data. It was seen that STAI-T and NSS values were significantly higher in Group S when compared to Group K. It was found no correlation between abstinence time and STAI-S, STAI-T and NSS. Conclusion: It was concluded that trait anxiety level is higher in smokers than non-smokers and cessation of cigarette smoking a day before surgery does not affect preoperative anxiety.
