Özet

Amaç: Fentanil, indüksiyon öncesi sıklıkla kullanılan bir ajandır ve zaman zaman öksürüğe neden olduğu bilinmek tedir. Fentanile bağlı öksürük konusunda mevcut literatürde büyük farklılıklar bulunmasının yanı sıra yapılmış klinik ça lışmalar da birbirleriyle çelişmektedir. Çalışmamızın amacı, indüksiyon öncesi kullanılan 2 µg/kg dozda fentanilin farklıinjeksiyon hızlarının Türk halkında oluşturduğu fentanile bağlı öksürüğün insidansını ve şiddetini belirlemekti.Gereç ve Yöntem: Lokal etik kuruk kararı ve hastalardan yazılı onamları alındıktan sonra elektif cerrahiye girecek ASAI-III 210 hasta bu randomize plasebo kontrollü çalışmaya alındı. Bronşial astım, kronik obstrüktif akciğer hastalığı, üstsolunum yolu infeksiyonu olanlar ile sigara içenler ve son 2 hafta içinde anjiotensin dönüştürücü enzim inhibitörü almışolanlar çalışma dışı bırakıldı. Hastalara cerrahi öncesi premedikasyon uygulanmadı. Hastalara 2 µg/kg fentanil 2 sn,20 sn veya aynı volümde % 0.9 NaCl 2 sn’de injekte edildi ve gruplar sırasıyla I, II ve III olarak belirlendi. Injeksiyonlartamamlandıktan sonra oluşan öksürüklerin başlama zamanı ve şiddeti 1 dk. süreyle bağımsız bir gözlemci tarafındankaydedildi. Abstract

Objective: Fentanyl is commonly used as a pre-induction adjunct, but can sometimes elicit cough. The existingliterature about the incidence of fentanyl-induced cough (FIC) is extremely diverse and the results of clinical trials areconflicting. The purpose of our study was to investigate the effects of intravenous pre-induction doses (2 µg/kg) offentanyl given with different injection velocities on the incidence and severity of FIC in Turkish population.Material and Methods: Following local ethical approval and obtaining written informed consents of the patients,210 ASA I-III, 18-75 year- old patients undergoing general anesthesia for elective surgery were enrolled into thisrandomized placebo- controlled study. The exclusion criteria included, history of bronchial asthma or chronic obstruc tive lung disease, upper respiratory tract infection, smoking, and angiotensin converting enzyme inhibitor use. Nopremedication was used before surgery. Patients received either 2 µg/kg fentanyl injected over 2 sec, 20 sec or thesame volume saline placebo (NaCl % 0.9) injected over 2 sec in Groups I, II, and III respectively. After completionof injections, the onset time and intensity of coughing was recorded by a blinded observer for 1 minutes.
