Karaciğer yetersizliğinde uygulanan ekstrakorporeal destek sistemlerinin etkileri

Özet
Amaç: Ekstrakorporeal destek sistemleri, karaciğer yetersizliğinde önemli bir tedavi seçeneği sunmaktadırlar. Bu retrospektif çalışmada, iki farklı destek sis- temi ile ilgili deneyimler ışığında bu sistemlerin etkileri araştırılmış ve sistemler, birbiriyle karşılaştırılmıştır. Yöntemler: Molekül abzorbe edici resirkülasyon sistemi (MARS) (10 hasta) ve plazmadan emilim yoluyla doğrudan adzorpsiyon (FPSA) (28 hasta) sistemleri- nin uygulandığı 38 hastada, 114 işlem değerlendirilmiştir. Karaciğer nakli ve sağ- kalım sıklığı araştırılmış; SOFA ve hepatik ensefalopati skorları, total bilirübin, amonyak, albümin, kan idrar nitrojeni (BUN), kreatinin, laktat düzeyleri, INR ve lökosit ve trombosit sayıları ve ayrıca hemodinamik değişikliklerin sıklığı in- celenip karşılaştırılmıştır. Kullanılmış olan antikoagülasyon yöntemi, destek sis- teminin kaç kez uygulandığı ve hastanın yoğun bakım kalış süresi kaydedilmiştir. Bulgular: Her hastaya 3±1,8 (sınır: 1-7) seans uygulanmıştır. Nakil yapılan 7 (MARS: 3; FPSA: 4) hastanın tümü yaşamış; nakil yapılmayan hastaların 12’si (%30,8) yaşamış; genel sağkalım %50 olmuştur. Her iki sistem ile bilirübin, BUN ve kreatinin düzeylerinde düşmeler sağlanmış; FPSA ile bu düşüşler daha belirgin olmuş; ayrıca amonyak düzeyinde de anlamlı azalma görülmüştür. FPSA ile albümin, trombosit ve lökosit sayısı ile INR’de de anlamlı değişiklikler saptanmıştır. MARS ile anlamlı olarak daha az hemodinamik sorun ile karşıla- şılmıştır. Sonuç: İki sistem de, karaciğer nakline kadar vakit kazanılmasında, nakil gerçek- leşene kadar hastanın genel durumunun düzeltilmesinde, yaklaşık 1/4 hastada da karaciğer nakli endikasyonunun ortadan kaldırılmasında etkili olabilmektedir. FPSA, detoksifikasyon işleminde daha etkin görünmektedir; buna karşın MARS, bu kritik hasta grubunda, daha istikrarlı bir hemodinami sağlayabilmektedir.
Abstract
Objective: Extracorporeal liver support systems represent an important option for the treatment of liver failure. In this retrospective study, effects of two differ- ent support systems were examined, and these systems were compared. Methods: Molecular Absorbent Recirculating System (MARS) (10 patients) and fractionated plasma separation and adsorption (FPSA) (28 patients) were performed 114 times in 38 patients. The frequencies of liver transplantation and survival; SOFA and hepatic encephalopathy scores, levels of total bilirubin, am- monia, albumin, blood urea nitrogen (BUN), creatinine and lactate, leucocyte and thrombocyte counts, as well as frequency of hemodynamic instability were investigated and compared. The method used for anticoagulation, the number of applications/patients and the length of stay in ICU were also recorded. Results: There were 3±1.8 (range: 1-7) treatments/patient. All transplanted patients (n=7; MARS: 3, FPSA: 4) and 12 (30.8%) of the remaining patients without transplantation survived, obtaining an overall survival of 50%. There was a decrease in the levels of bilirubin, BUN and creatinine with both systems; the decrease was more pronounced with FPSA. FPSA also achieved a significant decrease in ammonia level. With FPSA, there were also significant changes in albumin, leucocyte and thrombocyte counts and INR. MARS was associated with significantly less hemodynamic instability. Conclusion: Both systems have been found to be effective as a “bridge therapy” for liver transplantation, in 1/4 of the patients they can even eliminate the indi- cation for liver transplantation. Effects of FPSA on detoxification appeared to be more pronounced, whereas MARS obtained a more stable hemodynamic status during treatment.
