Sakrokoksigeal teratomlu yenidoğanda ameliyat esnasında anestezi yönetiminin önemi

Özet
Sakrokoksigeal teratom (SKT), yenidoğanlarda en sık karşılaşılan germ hücreli tümördür. Etyolojisi tam olarak bilinmeyen bu teratomların büyükboyutlara ulaşabilmesi, fazla kanlanması nedeniyle, operasyon esnasında cer-rahi ve anestezi yönetimi açısından sorunlara neden olabilmektedir. Prenataldönemde ultrasonografi ile SKT tanısı konulan ve 40. haftada C/S ile, 4050 gr olarak dünyaya gelen yenidoğanın fizik muayenesinde sakrokoksigeal bölgede 15x10 cm büyüklüğünde solid kitle dışında özellik yoktu. Preope-ratif tetkikleri normal sınırlarda bulunan olguya hayatın 2. gününde kitleeksizyonu yapıldı. İnvazif olarak arter, ven basınçları ve sıcaklık monitöri- zasyonuyla takip edilen olguda, operasyon esnasında ciddi bradikardi, hipo- tansiyon ve hipoksemi gelişti. İnvazif monitörizasyonla yakın takip edilen, bu nedenle de değişikliklerin anında fark edildiği olgumuz etkin müdahaleile kısa sürede düzeldi. Bu yazıda yenidoğan döneminde opere edilen dev SKT’li olgularda operasyon esnasında uygulanan invazif arter ve ven basıncı monitörizasyonunun son derece önemli olduğunu vurguladık.
Abstract
Sacrococcygeal teratoma is the most common germ cell tumor in newborn infants. Teratomas with unknown etiology can cause problems in the man- agement of surgery and anaesthesia during operation since they can reach high pressure levels due to increased blood supply. A sacrococcygeal terato- ma was diagnosed with prenatal ultrasound and a 4050 g 40 week newborn, was born with C/S. The physical examination was unremarkable except for a solid mass the size of 15x10 cm in the sacrococcygeal region. Preoperative anaesthesia examination of the case was within normal limits and at the second day of her life the mass was excised. The invasive arterial and central venous pressures with esophageal temperature monitorization was assured and severe bradycardia, hypotension, and hypoxemia developed during the operation. With close invasive monitorization, the changes were realised immediately and treated effectively. The importance of invasive arterial and venous pressure monitoring in patients operated on during the neonatal period with giant sacrococcygeal teratoma, is highlighted in this report.
