Çoklu kot fraktürlü bir olguda kosta fiksasyonu ile yoğun bakımda kalma süresinin kısalması

Özet
Çoklu kot kırıklarında çok ciddi solunum sorunları ortaya çıkmaktadır. Bu nedenle cerrahi olarak göğüs duvarı stabilizasyonun bir çok avantajı bildirilmiştir. Özellikle mekanik ventilasyonun kısalıp, yoğun bakımda kalış süresinin azalması enfeksiyon gibi gelişebilecek komplikasyonların önlenmesi bakımından önemlidir. Burada 36 yaşında trafik kazası nedeniyle çoklu kot fraktürü olup ciddi solunum sıkıntısı olan hastanın kot fiksasyonu sonrası dramatik olarak solunum paterninin düzelip ventilatör süresinin kısaldığı tartışılmıştır. Bu olgu sonucunda doğru endikasyon ile kosta fiksasyonu yapılan hastaların daha erken ve komplikasyonsuz olarak taburcu olduğu kanısına vardık.
Abstract
Severe breathing problems arise in multiple rib fractures. As a result, many advantages of the surgical stabilization of chest wall have been reported. Especially shortening mechanical ventilation along with a decrease in the duration of Intensive Care Unit stay is important for the prevention of possible infectional complications. In this study, the dramatic clearance of breathing pattern as well as the reduction in ventilator duration after rib fixation time of a 36 year old patient with severe respiratory distress who has multiple rib fractures due to road traffic accident was discussed. Due to this fact, it is concluded that patients could be discharged from the hospital earlier and uncomplicated as a result of the fixation of the rib with the right indications.
