Weaning' Modeli olarak basınç destekli (PSV) ve volüm destekli (VSV) ventilasyon modellerinin karşılaştırması

Özet
AMAÇ: Çalışmamızın amacı; weaning yöntemi olarak basınç hedefli destekli ventilasyon (PSV) ile volüm hedefli destekli ventilasyon (VSV) modellerinden, birinin diğerine üstünlüğünün olup olmadığını tespit etmektir. YÖNTEMLER: Çalışmamıza 60 hasta alınmıştır. Hastalar rastgele iki eşit gruba ayrılmıştır. PaCO2: 35-45 mmHg, pH>7,32, 6-8 mL kg-1 soluk hacmi (TV), satürasyon>%92, PaO2 >60 mmHg olacak şekilde, birinci gruba; FiO2 ve hava yolu basıncı ayarlanmış, PSV uygulanmış, ikinci gruba; FiO2, TV ve üst basınç seviyesi ayarlanmış, VSV uygulanmıştır. Hastalara her sabah T-parçası ile spontan solunum denemesi yapılmıştır. Spontan solunum denemesi sorunsuz olan hastalar ekstübe edilmişlerdir. Başarısız olan hastalarda mekanik ventilasyona yeniden başlanmıştır. Destekli ventilasyon süresi (DVS), mekanik ventilasyon süresi (MVS), toplam T-parçası süresi (TTS), toplam weaning süresi (TWS), sedasyon ihtiyacı (Sİ) kaydedilmiştir. İstatistiksel değerlendirmede T-Test, Ki-kare testleri kullanılmıştır. BULGULAR: Çalışmamızda ortalama DVS; PSV grubunda 82,60 saat, VSV grubunda 56,03 saat (p=0,041), ortalama TWS PSV grubunda 95,30 saat, VSV grubunda 61,53 saat (p=0,035), ortalama TTS PSV grubunda 7,67 saat, VSV grubunda 3,83 saat (p=0,007), bulunmuştur. Weaning süreci boyunca PSV grubunda 19, VSV grubunda 9 hastada Sİ olmuştur (p=0,01). Değerlendirilen diğer parametrelerde istatistiksel olarak anlamlı bir fark gözlenmemiştir. SONUÇ: Sonuç olarak weaning sürecinde kullanılan VSVnin PSVye daha üstün olduğuna ilişkin ön bulgular elde edilmiştir.
Abstract
OBJECTIVE: The purpose of our study is to compare two different ventilation modes-pressure support ventilation (PSV) and volume support ventilation (VSV)-as the means of weaning. METHODS: Sixty patients were enrolled in our study. Patients were randomized in to two groups. For the PSV group, FiO2 and airway pressure values were adjusted in order to sustain PaCO2: 35-45 mm Hg, pH>7.32, 6-8 mL kg-1 TV (tidal volume), and saturation >92%. For the VSV group, FiO2, TV, respiration frequency (f), and peak pressure were adjusted to obtain PaCO2: 35-45 mm Hg, pH>7.32, 6-8 mL kg-1 TV, saturation >92%, and PO2>60 mm Hg. Every morning, spontaneous breathing was tried in those patients. The patients were extubated after 2 hours of T-piece breathing. The patients who failed spontaneous respiration with the T-piece were returned to mechanical ventilation. Assisted respiration time (ART), mechanical respiration time (MRT), total T-piece time (TTT), total weaning time (TWT), and sedation need (SN) values were recorded.
