Astma atakları sırasında "Ötiroid hasta sendromu"

Özet
Nontiroidal birçok hastalık tiroid hormon metabolizmasında değişikliğe neden olabilmektedir. Düşük T3 ve T4 değerlerine rağmen, normal serum TSH değerine sahip bu olgular ötiroid olarak kabul edilirler. "Ötiroid hasta sendromu" (Euthyroid Sick Syndrome) adı verilen bu tablo tiroid hormon metabolizmasının nontiroidal hastalığa bir adaptasyonu olarak bilinir. Çalışmamızda astma atakları sırasındaki tiroid hormon değişiklikleri ve bu sendromun varlığı araştırıldı. Bu amaçla astma atağı ile kliniğimize başvuran 52 olgu çalışmaya alındı ve atağın ağırlığı hafif, orta ve şiddetli gerekirse bilgisayarlı tomografisi, tiroid antikorları ve tiroid hormonları bir endokrinolog tarafından değerlendirildi. Primer hastalığı saptanan 12 olgu çalışma dışı bırakıldı. Geri kalan 40 olgunun (10 erkek, 30 kadın) astma atağı başlangıcında ve sonunda tiroid hormon düzeylerine (T3, ST3, T4, ST4, TSH) bakıldı. Astma atağı sırasında hastaların 11'inde ST3 ve 3'ünde ST4 değerlerinin düşük olduğu görüldü, TSH düzeyleri normaldi. Düşük ST3 değerleri hafif astma atağı olanların % 12, orta dereceli atağı olanların ise % 53'ünde saptandı. Astma atağı sonrasında tüm hastaların tiroid hormonları normal düzeylerde bulundu. Çalışmamızda, astma atağı sırasındaki tiroid fonksiyon bozuklukluğunun atağın şiddetiyle ilişkili olduğu ve bu hormonal değişikliklerin şiddetli nontiroidal hastalık, yani "ötiroid hasta sendromu" na bağlı geliştiği sonucuna varıldı,
Abstract
"EUTHYROID SICK SYNDROME" DURING"ASTMA ATTACKS Many nonthyroidal illness in adults cause alterations in thyroid hormone metabolism. Despite depressed serum T3 and T4 levels, these patients are considered to be euthyroid as evidenced by normal basal serum TSH levels. This is called the "Euthyroid Sick Syndrome" that is known as an adaptation to nonthyroidal illness. The aim of this study was to evaluate the presence of this syndrome and thyroid hormonal changes during asthma attacks. Fifty-two patients with asthma attacks were included in the study. The clinical severity of asthma attacks was assesed as mild, moderate and severe. The study protocol included; thyroid examination by an endocrinologist, ultrasonography, scannig, computed tomography (as indicated) and serum antibodies of the thyroid. In addition, serum T3, T4 Free T3, Free T4 and TSH levels were measured before and after the treatment of asthma attacks. 12 patients were excluded from the study because of their primary thyroid disease. The remaining 40 patients (10 men and 30 women) constituted the study group. At the onset of asthma attacks, decreased levels of FT3 and FT4 were observed in 11 and 3 of the patients respectively, TSH levels were normal. Decreased FT3 levels were found in 12 % and 53 % of the patients with mild and moderate asthma respectively. After the treatment 'of asthma attacks, the levels of thyroid hormones were within the normal range in all the patients.Our findings suggest that thyroid dysfunction during asthma attacks correlates with the severity of asthma and thyroid hormonal changes, are attributed to the "euthyroid sick syndrome" that is typical for severe nonthyroidal illnesses.
