Özet
Nokardiozis, aerobik aktinomiçes grubu Nocardia asteroides'in oluşturduğu granülomatöz, süpüratif infeksiyon hastalığıdır. Pulmoner nokardiozis nadir görülür, tanınması güçtür. Bildirdiğimiz pulmoner nokardia olgusu 1987'de böbrek transplantasyonu yapılıp, 1994'te transplante böbrekte rejeksiyon gelişen hemodializ hastasıdır. Onbeş gündür kuru öksürük, akşam yükselen ateş, nefes darlığı yakınmaları olup, rejeke böbreğin çıkartılması için Genel Cerrahi Servisi'ne yatırılmıştır. Fırsatçı pulmoner infeksiyon düşünülüp, uygulanan empirik antibiyotik tedavisine yanıt alınmamıştır. Rutin yöntemlerle tanı konulamamış, etken izole edilememiştir. Yatak başı mini torakotomi ile alınan açık akciğer biyopsisi materyalinden histopatolojik olarak nokardiozis tanısı konulmuştur. Uygun tedavi almasına rağmen ağır bir dissemine intravasküler koagülasyon (DIC) tablosu ile olgu kaybedilmiştir. Fırsatçı bir infeksiyon olarak karşımıza çıkan pulmoner nokardiozisin ender görülmesi, tanı güçlüğü, mortal klinik seyri ve tanıda gerekirse invaziv yöntemlere başvurulması gerektiğini vurgulamak amacıyla olgu sunulmuştur.
Abstract
Nocardiosis is a granulomatous, suppurative infectious disease produced by Nocardia asteroides of aerobic species. Pulmonary nocardiosis is a difficult to define and rarely seen disease. At 1987 the patient had a renal transplantation but at 1994 this transplanted kidney was rejected and hemodialysis was started. During the past fifteen days before the admission he had complains of dry cough, fever during nights and shortness of breath. There was no response to the ampirical antibiotic treatment that was started assuming he was having an oppurtunistic pulmonary infection. Through the routine and conseruative methods applied, no diagnosis had been obtained nor the infectious agent could be isolated. Nocardiosis diagnosis was reached by obtaining an open lung biopsy material done through a bedside minor thoracotomy. Even though having an appropriate treatment, the case was lost by a severe disseminated intravascular coagulation condition. This pulmonary nocardiosis case appearing as an oppurtunistic infection is presented because it is a rarely seen, difficult to define disease, and to recognise that it may be have a fatal clinical progress, and if necessary invasive methods should be used to obtain an exact diagnosis.
