Diğer yöntemlerle tanısı konulamamış plevral efüzyonlu hastalarda video yardımlı torakoskopik cerrahi (VATS)'nin rolünü ve tanı değerini göstermek istedik. Ocak 1997-Temmuz 2000 tarihleri arasında VATS dışı (torasentez ve kapalı pleura iğne biyopsisi) yöntemlerle tanısı konulamayan 28 plevral efüzyonlu hastaya VATS yapıldı. Olgular retrospektif olarak incelendi. Olgulardan 17 (60.7)'si erkek, 11 (%39.3)'i kadındı. Yaş ortalaması 50.8 (30-72) idi. Olguların 2'si hariç, tümüne çift lümenli endotrakeal entübasyon ile işlem yapıldı. Bütün hemitoraks boşluğu gözlendikten sonra gerekli yerlerden biyopsi örnekleri alındı. En sık izlenen makroskobik görüntü 13 olgu ile difüz pleura kalınlaşması idi. En sık karşılaşılan klinik semptom 23 olgu ile dispne idi. Olgulardan 13'ünde aspest teması mevcuttu. Patolojik sonuç olarak, olguların 15'i maligndi ve 9'u malign mezotelyoma idi. Tüp drenajı ortalama 7 gündü. Postoperaüf komplikasyon oram %10.7 idi. VATS, tüm toraks boşluğunu araştırmak ve birçok cerrahi işlemi yapabilmek için çok elverişlidir. VATS, tanısı konulamayan pleural efüzyonlu olgularda %96.5 oranında yüksek tanı değeri ue düşük komplikasyon oranı ile güvenli bir yöntemdir.
Abstract
We wanted to demonstrate the role and diagnostic value of video-assisted thoracoscopic surgery (VATS) in patients with nondiagnostic pleural effusions. VATS was performed in 28 patients with nondiagnostic pleural effusions, who could not be diagnosed except VATS (thoracentesis and closed pleural needle biopsy). Patients were examined retmspectiuely. Seven-teen patients (60.7%) were male and eleven (39.3%) were female. Median age was 50.8 (30-72). All but two patients were undertaken with double lumen endotracheal tube. After the whole hemithorax was observed than biopsy materials were taken wherever necessary. The most common macroscopic view was diffuse pleural thickening with thirteen patients. The most common clinical symptom was dyspnea with twenty-three patients. Thirteen patients had asbestos exposure. Patho-logical results were malign in 15 patients, 9 of whom had malign mesothelioma. Mean chest tube drainage time was 7 days. The incidence of postoperative complication was 10.7%. VATS is very useful to investigate the whole pleural cavity and per-form a lot of surgical procedures. VATS is a safe method with low rate of complication and high diagnostic value as 96.5% in patients with nondiagnostic pleural effusions.
