Similarity and differences of tuberculosis in the young and the elderly patients

Özet

Genç ve yaşlı hastalarda tüberkülozun benzer ve farklı yönlerini saptamak için, 1995-1999 yılları arasında kliniğimizde izlenen 123 olgunun dosyalarını retrospektif olarak inceledik. Bu amaçla, 35 yaş ve altı 87 olgu (grup 1) ve 55 yaş ve üzeri 36 olgunun (grup 2) verileri karşılaştırıldı. Olgularımızın tümü erkek olup, yaş ortalaması grup 1 'de 25 ± 5.75, grup 2'de 59 ± 3.18 idi. Semptomlardan yalnızca göğüs ağrısı grup 1 'de daha fazlaydı (p> 0.05). Öksürük, balgam çıkarma (p 0.05) grup 2'de sıktı. Grup 1'de tüberküloz teması %20.6, eşlik eden hastalık %3.4, geçirilmiş tüberküloz öyküsü %2.2, asidorezistan basil (ARB) pozitifliği %59.7 bulundu. Bunlar grup 2'de sırasıyla %13.8, %38.8 (p< 0.05), %25 (p= 0.00043), %80.5 (p< 0.05) idi. Gençlerle benzer semptomları sergilemesine karşın, eşlik eden diğer hastalıklar nedeniyle atipik klinik görünüme sahip yaşlılarda, ayırıcı tanıda tüberkülozun da düşünülmesi gerektiği sonucuna varıldı.

Abstract

To determine the similarity and differences of tuberculosis in young and the elderly patients, we investigated the records of 123 subjects, who were followed up in our clinic between 1995-1999. For this purpose, group 1 (n= 87, 35 years old and younger), and group 2 (n= 36, 55 years old and older) were compared. All subjects were male, mean ages of group 1 and group 2 were 25 ± 5.75, 59 ± 3.18 respectively. Thoracic pain was higher in group 1 (p> 0.05), cough, sputum expectoration (p 0.05) were higher in group 2. In group 1, the ratio of contact with tuberculosis was 20.6%, accompanying disease 3.4%, history of tuberculosis 2.2%, presence, of acid-fast bacilli 59.7%. These ratios were 13.8%, 38.8% (p< 0.05), 25% (p= 0.00043), and 80.5% (p< 0.05) respectively in group 2. In conclusion, although they present similar symptoms as the young, in the elderly patients who have atypical clinical appearance because of other accompanying diseases tuberculosis must be also taken into account in differential diagnosis.
