Substernal guatrda yumurta kabuğu şeklinde kalsifikasyon

Özet

Yetmiş bir yaşında bayan hasta kliniğimize öksürük, ateş ve çarpıntı şikayeti ile başvurdu. Fizik muayenede; timid sag lobda yaklaşık 3x4 cm, istmusta 2 cm çapında nodul ele geliyordu. Hafif derecede kifozu olan hastada taşikardi saptandı. Diğer sistem muayeneleri normal olarak değerlendirildi. Çekilen akciğer grafisinde, üst mediasten genişti ve sol akciğer apeksinde yaklaşık 3.5 cm çapında etrafı kalsifiye nodul izlendi. Toraks bilgisayarlı tomografisi (BT)'nde; normalden büyük ve üst mediastene doğru uzanım gösteren tiroid bezi ve tiroid sol lob inferiorunda 3.5 x 3.2 cm boyutlarında çeperi kalsifiye nodul saptandı. Tiroid fonksiyon testleri hiperüroidiyi gösteriyordu. Klinik olarak hasta subslernal multinodüler guatr olarak değerlendirildi. Akciğer grafisinde sol apekste yumurta kabuğu şeklinde kalsifikasyon gösteren lezyonun substernal guatra ait olması nedeniyle olgu ilginç bulunmuş ve sunulmuştur.

Abstract

Seventy one years old woman admitted to our clinic with the complaints of fever, cough and palpitation. Physical examination revealed a palpabl thyroid with a 2 cm in length in its isthmus and large right lobe. Kyphotic patient was tachycardic. Other systems were within normal limits. Chest x-ray performed wide upper mediasten with calcified nodule approximately 3.5 cm in dimension in the apex of the left lung. Computed tomography revealed thyroid gland lying through the upper mediasten and a mass 3.5 x 3.2 cm in dimension in the left lobe inferior regio of the gland surrounded with calcification. Thyroid function tests showed hyperlhyroidism. Clinically patient had a substernal multinodular goitre. Since the lesion demonstrating egg shell calcification at the apex of left lung in the chest roentgenogram belonged to substernal goitre, the case was accepted interesting and presented below.
