KOAH'da hiperinflasyonun solunum kasları ve solunum paterni üzerine etkisi

Özet
Kronik obstrüktif akciğer hastalığı (KOAH)'nda solunum kas yorgunluğu ve gaz değişiminde bozulmalara neden olabilen akciğer hiperinflasyonu, hava yolu obstrüksiyonu, hava yolu rezistansında ve kompliyansında artma sonucunda gelişir. Bu çalışmadaki amacımız; hafif-orta ve ileri dereceli KOAH'da hiperinflasyonun solunum kas gücü, solunum paterni ve gaz değişimi üzerine olan etkilerini ve gruplar arasındaki farklılıkları araştırmaktır. Radyolojik ve tomografik olarak amfizem bulgusu olan 28 KOAH'lı olgu çalışmaya alındı. Bu olgular KOAH'ın şiddetine göre gruplandırddı. Grup1= FEV1 $leq$ %49 (n= 16), Grup II= FEV1 $geq$ %50 (n= 12). Grup I'de hava yolu akım hızları belirgin azalmıştı ve hava yolu rezistansı artmış ve maksimal inspiratuar basınç (MlP) önemli ölçüde azalmıştı. Her iki grupta da FRC, RV, RV/TLC oranı %120'nin üzerinde artmıştı. Grup I'de orta derecede hipoksemi (PaO2= 54.02 mmHg) ve hiperkapni (PaCO2= 46.65 mmHg) gözlenirken, Grup Il'de hafif derecede hipoksemi (PaO2= 63.78 mmHg) ve normokapni vardı. Solunum paterni her iki grupta benzerdi. Diyafragma kubbe yüksekliği indeksi (DHl) gruplar arasında anlamlı farklılık göstermezken, akciğer volümleriyle olan ilişkisine bakıldığında, RV ve FRC ile arasında anlamlı pozitif ilişki gösterildi. MlP ile hava yolu akım hızlan arasında pozitif, MlP ile DLCO arasında pozitif, MlP ile akciğer volümleri arasında negatif, MlP ile PaO2 arasında pozitif ve MlP ile PaCO2 arasında negatif bir ilişki vardı. FRC; Ti, Ti/Ttot ile negatif korelasyon gösteriyordu. Sonuç olarak hiperinflasyon, hafif derecede KOAH olgularında bile, inspiratuar kas güçsüzlüğüne ve buna bağlı gelişebilen gaz değişim bozukluklarına neden olabilir.
Abstract
Lung hyperinflation is a consequence of airway obstruction, increased airway resistance and compliance in patients with chronic obstructive pulmonary disease (COPD) which may result in respiratory muscle fatique and deterioration of gas transfer. The aim of this study was to investigate the influence of hyperinflation on respiratory muscles, gas transfer and breathing pattern and compare the differences between mild and severe COPD. Twenty-eight COPD patients with radiological and tomographic evidence of emphysema were included in the study and they were divided into two groups according to the severity of COPD. Group I= FEV1 $leq$ 49% pred (n= 16). Group II = FEV1 > 50% pred (n= 12). Airflow rates were dec-reased and airway resistance was increased significantly in Group I. Maximal inspiratory pressure (MIP) was significantly reduced in Group I. FRC, RV and RV/TLC ratio were increased above 120% pred in both groups with more significant inc-rease in Group I. Group I showed moderate hypoxemia PaO2 54.02 mmHg) with hypercapnia PaCO2= 46.65 mmHg) whereas Group II patients were mildly hypoxemic PaO2= 63.78 mmHg) with normocapnia. Parameters of breathing pat-tern were similar in both groups. Diaphragm height index (DHI) didn't showed significant difference between groups. But there were significant correlations between DHI and RV, FRC. MIP showed significant positive correlation with airflow rates and DLCO, negative correlation with lung volumes, positive correlation with PaO2 arid negative correlation with PaCO2 FRC also negatively correlated with Ti and Ti/Ttot. In conclusion, hyperinflation present even in the mild forms of COPD causes inspiratory muscle weakness which in return results in impairment in gas transfer.
