Özet

Bu çalışmada, klinik ve radyolojik olarak akciğer tüberkülozu (Tbc) düşünülen hastalarda; aktif pulmoner Tbc'nin yüksek rezolüsyonlu bilgisayarlı tomografi (YRBT) bulgularını tanımlamak, YRBT'nin aktif ve inaktif Tbc'nin ayrımında yerini, belirlemek amaçlanmıştır. Aktif Tbc'li 67 hastanın 66 (%98.5)'sında sentrilobüler nodul veya dallanan lineer yapılar, (%97)'inde asiner nodul, 55 (%82)'inde konsolidasyon, 55 (%82)'inde kavite, 54 (%80.5)'ünde tomurcuklu dal manzarsı 52 (%77.6)'sinde bilateralite saptanmıştır, inaktif Tbc'li 30 hastanın 26 (%86.6)'sında fibrotik değişiklikler, 20 (%66.6)'sin bronşektazi, 18 (%60)'inde bronkovasküler distorsiyon, 14 (%46.6)'ünde periskatrisyel amfizem saptanmıştır. Hastalık aktivitesini belirlemede toraks YRBT'nin sensitivitesi %97, spesifısitesi %86.7, pozitif prediktif değeri %94.2, negatifprediktifli geri %92.9 olarak tespit edilmiştir. Sonuç olarak; toraks YRBT'nin akciğer Tbc'si tanısında güçlü oe güvenilir bir tanı yöntemi olduğunu söyleyebiliriz. Toraks YRBT, özellikle yayma ve kültür negatif aktif Tbc hastalarında noninuaziv bir tanı yöntem olarak kullanılabilir.

Abstract

In this study we have tried to put forth the role of thorax high resolution computerized tomography (HRCT) in the purs of the diagnosis and treatment of pulmonary tuberculosis. It was detected that of the 67 patients with active tuberculosis 66 (98.5%) had centrilobular nodule or branching lineer structures, 65 (97%) had acinary nodule, 55 (82%) had consolidation , 55 (82%; had cavities, 54 (80.5%) had tree in bud apperance and 52 (77.6%) were bilateral. Of 30 patients suffen from inactive tuberculosis, 26 (86.6%) were detected to have fibrotic changes, 20 (66.6%) bronchiectasis, 18 (60%) broncovascular distortion and 14 (46.6%) pericicatrial emphysema. The sensitivity, spesifity, positive predictive value, negative predictive values of thorax HRCT in determining the activity of the illness were found as 97%, 86.7%, 94.2% and 92.9% respectively. In conclusion we can say that thorax HRCT is a powerful and reliable diagnostic method for pulmonary tuberculosis. Thorax HRCT can be used as noninvasive diagnostic method especially in the patients suffering from smear and culture negative active tuberculosis.
