Successful treatment of a huge thrombus with thrombolytic therapy in a patient with normal left ventricle function and Takayasu arteritis

Özet

Bu yazıda, normal ventrikül fonksiyonuna sahip Takayasu arteritinde sol ventrikül içerisindeki dev trombüsüntrombolitik tedavi ile başarılı şekilde tedavisi sunuldu. Takayasu arteriti öyküsü olan hasta her iki bacağında güçsüzlük ve ağrı ile başvurdu. Doppler ultrason incelemesi her ikipopliteal arterde subtotal trombotik tıkanma ile uyumlu bulundu. Transtorasik ekokardiyografi incelemesinde normalsol ventrikül fonksiyonu ve boyutlarına rağmen sol ventriküliçerisinde dev trombüs tespit edildi. Daha kötü sonuç doğurabilecek embolilerden kaçınmak için hastaya cerrahi tedaviönerildi fakat hasta cerrahi tedaviyi kabul etmedi. Trombolitiktedavi uygulama kararı verildi. Üçüncü günde sol ventriküliçerisindeki trombüsün tamamen kaybolduğu ve her iki ayakdolaşımının da normale döndüğü gözlendi.

Abstract

We report a case of thrombus treated withthrombolytic therapy in a patient with normal cardiac func- tions and Takayasu arteritis. A 27-year-old man with a historyof Takayasu arteritis was admitted to our out-patient clinicwith a complaint of both right and left foot pain and weak- ness. In a Doppler ultrasound examination, a subtotal throm- botic occlusion was found in the bilateral popliteal arteries.A transthoracic echocardiography revealed a left ventricularapical thrombus, although both cardiac functions and heartdimensions were normal. Surgical excision was recommend- ed to avoid further embolization, but this was refused by thepatient. We decided to perform thrombolytic treatment. Thethrombus in the left ventricle was fully resolved by the thirdday. Perfusion in both feet also returned to normal.
