Laparoskopik Apendektomi ve Açık Apendektominin Yaşam Kalitesi Üzerine Etkisi

Özet

Amaç: Eylül 2014-Mart 2015 tarihleri arasında acil servise başvuran ve preoperatif klinik ve radyolojik olarak akut apandisit tanısı konan hastalarda uygulanan cerrahi prosedürün yaşam kalitesi üzerine etkisini araştırmak..Yöntem: Bu çalışma Eylül 2014-Mart 2015 tarihleri arasında acil servise başvuran ve preoperatif klinik ve radyolojik olarak akut apandisit tanısı konan 53 hastaya laparoskopik ya da açık apendektomi yapıldıktan sonra postoperatif 1. gün ve 1. aylarda SF 36 anketleri uygulanarak yapılmıştır.Bulgular: Çalışmaya 53 hasta katıldı. 33'ü(%62,3) kadın, 20'si (%37,7) erkek olup 21'ine (%39,6) açık, 32'sine (%60,4) laparoskopik prosedür uygulandı. Postoperatif analjezik ihtiyaçlarına bakıldığında her 2 grup arasında 0-1 ve 2. saatler arasında anlamlı fark saptanmazken postoperatif 3. saatte 2. grubun analjezik ihtiyacının giderek azaldığı gözlendi. SF 36 anketlerinin değerlendirilmesinde; postoperatif 1. günlere bakıldığında ruh sağlığı, fiziksel fonksiyon, fiziksel rol ve sosyal fonksiyon açısından gruplar arasında istatistiksel farklılık saptanmazken,ağrı,genel sağlık algısı,canlılık, emosyonel rol, fiziksel bileşen özet ve mental bileşen özetleri açısından laparoskopik apendektominin açık apendektomiye göre istatistiksel olarak anlamlı olduğu görüldü. 1. ay anketlerinde ise sosyal fonksiyon açısından istatistiksel olarak anlamsız bulunurken,fiziksel fonksiyon, fiziksel rol, ağrı, genel sağlık algısı, canlılık, emosyonel rol, ruh sağlığı, fiziksel bileşen özet ve mental bileşen özetleri açısından laparoskopik apendektomi puanları açık apendektomiye göre istatistiksel olarak anlamlı bulundu. Abstract

Purpose: WE investigated effect of surgical procedure open vs.laparoscopic on qulaity of life in patients who admitted to the emergency department between September 2014-March 2015 and diagnosed with acute appendicitis clinically and radiologically. Materials and Methods:SF-36 questionnaires was applied to 53 patients in first postoperative day and first postoperative month after laparoscopic or open appendectomy who was admitted to the emergency department between September 2014-March 2015. Results: 53 patients participated in the study. 33 (62.3%) of participants were female, 20 (37.7%) of participants were male, 21 (39.6%) of participants underwent open procedures, 32 (60.4%) underwent laparoscopic procedure. When we compared postoperative analgesic requirements, there was no significant difference between zero-first and second hours between 2 groups, but the analgesic dose of the second group was observed to decrease gradually at third postoperative hour. SF-36 questionnaires at first postoperative days, there was no significant difference between groups for mental health, physical function, physical role and social function, but laparoscopic appendectomy was found to have statistically significant higher scores compared to open appendectomy for pain, general health perception, vitality, emotional role, physical component summary and the mental component summaries. In questionnaires of the first month, while there was no statistically significant difference between groups for social function, laparoscopic appendectomy points were found to be higher statistically significant compared to open appendectomy points for physical function, physical role, pain, general health perception, vitality, emotional role, mental health, physical component summary and the mental component summaries. Conclusion: Better SF-36 results was found in laparoscopic procedure which had a positive impact on quality of life in the early and late term. ( Sakarya Med J 2016, 6(2):67-72 )
