Özet

Serum karbonhidrat antijeni 19-9 (CA 19-9) tümör ilişkili glikosfingolipid yapıda bir antijendir. CA 19-9 ölçümüne yönelik uluslararası standart bir yöntem yoktur. İmmunoassay ölçümlerde, plazma proteinler, heterofil antikorlar, hemoliz, ilaç metabolitleri gibi etkenler interferansa neden olabilirler. Bu yüzden farklı ölçümlerde farklı tetkik sonuçları elde edilmesi olasıdır. Ancak tedavi sonrası kür elde edilmişse serum CA 19-9 düzeylerinin tedavi öncesine göre azalması hastanın takibi için anlamlıdır. Bu olgu sunumunda, serviks uterinin malign neoplazmı tanısı ile tedavi olan ve tedavi sonrası laboratuvarımızda serum CA 19-9 seviyesi kliniği ile uyumsuz olarak yüksek ölçülen bir hasta irdelenmiştir. Hastada olası heterofil antikor interferansı düşünülmüş ve bunu saptamak için, hastadan tekrar numune alınarak heterofil antikor blokan tüp (HBT) ile serum CA 19-9 ölçümü tekrarlanmıştır. Tekrarlanan bu ölçümde CA 19-9 seviyesi bu kez hastanın kliniği ile uyumlu olarak düşük seviyede tespit edilmiştir. Bu bulgu, immunoassay ölçümlerde klinikle uyumsuz sonuç elde edildiğinde; heterofil antikorlardan kaynaklanabilecek interferansın akılda tutulması gerektiğini göstermiştir. . Abstract

Serum carbohydrate antigen 19-9 (CA 19-9) is a tumor-associated glyphospholipid antigen. There is no international standard method for CA 19-9 measurement. In immunoassay measurements, factors such as plasma proteins, heterophile antibodies, hemolysis, drug metabolites may cause interference. Therefore, it is possible to obtain different test results at different measurements. However, if cure was achieved after treatment, decrease in serum levels of CA 19-9 compared to pretreatment was significant for the patient’s follow-up. In this case report, a patientwho was treated with malignant neoplasm of cervical uterus and whose serum CA 19-9 level was inconsistent with our clinic was evaluated. Possible heterophile antibody interference wasconsidered in the patient and serum CA 19-9 measurement was repeated with heterophile antibody blocking tube (HBT). In this repeated measurement, the level of CA 19-9 was found to below at this time consistent with the patient’s clinic. This finding was found to be incompatible with the clinical results of immunoassay measurements; showed that interference from heterophileantibodies should be kept in mind.
