Acil Serviste Tanı Atlanmasına Neden OlanHatalar ve Zorluklar

Özet

Eksik veya hatalı tanı acil serviste sık karşılaşılan bir durumdur. Bir hastanın ilk muayenesinde konulamayan tanı daha sonraki süreçte tespit edilirse, atlanmış tanıdan bahsedilir.Bu çalışmanın amacı, kırık ve çıkıklarda tanı atlama ile ilgili literatürün gözden geçirilmesi ve konuya yönelik öneriler sunmaktır. Acil servislerde daha fazla sayıda hastanın daha tecrübeli doktorlar tarafından görülmesi ve tecrübesiz doktorların daha iyi denetlenmesi, doktorların çalışma ortam ve sürelerinin, uyku düzenlerinin optimal düzeyde olması sağlanmalıdır. Hataların azaltılması hedeflenerek eğitim programlarının uygunluğu ve kalitesi arttırılmalıdır. Dikkatli, detaylı öykü ve fizik muayene standardizasyonu sağlanmalıdır. Abstract

Incomplete or incorrect diagnosis is common in Emergency Room settings.A missed diagnosis is defined as no diagnosis made on first examination of a patient and that the diagnosis is made at a later date. The aim of this study was to review the lite rature related to missed diagnoses of fractures and dislocations, and to present recommendations. As a result of the review it can be concluded that more patients in EmergencyDepartments should be seen by more experienced doctors and there should be better supervision of inexperienced doctors. The working hours, environment, and sleep regime ofphysicians must be optimal. The appropriateness and quality of training programs should be improved with the targeting of error reduction. For all patients, a careful, detailedhistory must be taken and a standardized physical examination applied
