Özet

Pnömoni, Hindistan gibi tropikal ülkelerde en sık görülen solunumsal problemdir. Allerjik bronkopulmoner aspergillozis (ABPA), pnömoni ve tüberküloz gibi solunumsal hastalıklar ile klinik ve radyolojik olarak karıştırıldığı için genellikle tanıda ve incelemelerde atlanmaktadır. ABPA, uzun süreli atopik astmalı hastalarda Aspergillus antijenine aşırı duyarlılık ile karakterize, aspergillus mantarına ait en iyi tanımlanmış bir tablodur. ABPA’nın hastaneye yatırılan astmatik hastaların %20’sinde ve tüm rinit olgularının %5’inde çeşitli klinik tablolarla ortaya çıktığı bildirilmiştir. Burada, öksürük, ateş, nefes darlığı gibi yakınmaları ile tekrarlayan pnömoni tanısı ile tedavi edilen, birkaç yıldır astım olduğu bilinen orta yaşlı erkek olgu sunulmuştur. Bu olguda, sonuçta ABPA olarak tanı konmuş ve sistemik steroid ve antifungal tedavi ile tam klinik ve radyolojik iyileşme sağlanmıştır.

Abstract

Pneumonia is the most common respiratory problemin tropical countries like India. Allergic bronchopulmonary aspergillosis (ABPA) is routinely underdiagnosed and under evaluated as a result of clinical andradiological overlap with many other respiratoryconditions, including pneumonia and tuberculosis.ABPA is the best-recognized manifestation of thefungus Aspergillus, associated with hypersensitivity toAspergillus antigens in patients with longstandingatopic asthma. ABPA has been reported to occur in20% of asthmatic patients admitted to hospitals andin 5% of all rhinitis cases, with varied clinical presentations. This report is a description of the case of amiddle-aged male with known asthma for severalyears and constitutional symptoms, such as a cough,fever, and shortness of breath, who was diagnosedand treated for recurrent pneumonia. The case waseventually confirmed as ABPA, and a complete clinical and radiological response to medical treatmentwith antifungals and systemic corticosteroids wasdocumented.
