Özet

Bu yazının amacı, sosyal güvenlik harcamalarının ikinci büyük kalemini oluşturan ilaç harcamaları alanında yürütülmekte olan kamu politikalarına ilişkin tanımlayıcı ve açıklayıcı bazı tespitlerde bulunmaktır. Bu alan 2000li yılların başından itibaren özellikle AKP Hükümetinin, sağlık alanının neoliberal yeniden yapılandırılmasının aracı olan Sağlıkta Dönüşüm Programı ile artan bir şekilde görünür hale gelmiştir. Yazıda, özellikle kamunun artan ilaç harcamaları ile bağlantılı olarak ilaç piyasasında faal olan aktörlere ve süreçlere yönelik yürütülen politikalar ve düzenlemeler, devletin neoliberal yeniden yapılandırılma süreci bağlamında ilaç piyasasına müdahale biçimlerinin dönüşümü üzerinden tartışılacaktır. Bu kapsamda 2009 yılı ile birlikte bu alandaki en temel düzenleme araçlarından biri haline gelen global bütçe uygulamalarının devlet/sermaye ilişkilerinin yeniden tanımlanması noktasındaki rolü gösterilmeye çalışılacaktır.

Abstract

The aim of this article is to present descriptive and interpretive explanations regarding the public policies conducted in the field of pharmaceutical expenditures, which is the second big item of social security expenditures. This field, since the beginning of 2000s, particularly with the Health Transformation Program, that is the policy tool for neoliberal restructuring of health by the governments of Justice and Development Party, becomes specifically visible. In this text, the policies about the actors and process in the pharmaceutical market, which are connected to the rising of public pharmaceutical expenditures, will be discussed in the context of neoliberal structuring of the state through the changing modes of intervention to the market. Within this context, the role of "global budget" applications, as the main regulatory policy tool within the field since the year 2009, in the process of redefinition of the relations between state and capital will be explained.
