İskelet Kasında İskemi-Reperfüzyon Hasarı Uygulanan Sıçanlarda Karnosinin (Beta-Alanil-L-Histidin) Eritrosit Deformabilitesi Üzerine Etkisi

Özet

Amaç: İskemi‐reperfüzyon hasarının fizyopatolojisini açıklamaya yönelik çeşitli mekanizmalar ileri sürülmüştür. Eritrositlerin reolojik özel‐liklerindeki bozulma önemli hipotezlerden birisidir. Çalışmamızda doğal bir antioksidan ve serbest radikal temizleyici olan l‐car no si nin, rat‐ larda alt ekstremite iskemi‐reperfüzyon hasarı modelinde, eritrositlerin deformabilitesi üzerine etkisinin araştırılması amaçlanmıştır. Gereç ve Yöntem: Ratlar her biri 6 hayvandan oluşan 4 gruba randomize edildi: 1) İskemi‐reperfüzyon grubu, 2) Kontrol grubu (C), 3) L‐karnosin kontrol grubu (C‐C) ve 4) L‐karnosin + iskemi‐reperfüzyon grubu (C‐ iskemi‐reperfüzyon). Cerrahi öncesinde ketamin (90 mg/kg) + ksilazin (10 mg/kg) ile genel anestezi uygulandı. İskemi‐reperfüzyon uygulanacak olan ratlara orta hat laparotomi yapıldı. İnfrarenal abdominal aortaya ulaşıldı. Abdominal aortaya, atravmatik bir mikrovasküler klemp konuldu. 120 dakikalık iskemik periyodu takiben abdominal aortadaki mikrovasküler klemp kaldırıldı ve 120 dakika süre ile reperfüzyon sağlandı. Tüm gruplara işlemden 30 da‐ kika önce heparin (20 İÜ/kg) uygulaması yapıldı. İskemi‐reperfüzyon oluşturulan gruplara deney prosedüründen 1 saat önce 250 mg/kg dozunda l‐karnosin (intraperitoneal) uygulandı. Kontrol grubunu oluşturacak ratlarda laporotomi ve abdominal aortaya diseksiyonu eşit sürede (240 dakika) uygulandı ancak bu gruplarda iskemi‐reperfüzyon oluşturulmadı. Ratlar anestezi altındayken intrakardiyak kan alın‐ ması yöntemi ile sakrifiye edildi. Eritrositlerin deformabilite yeteneklerinin ölçümü için sabit akımlı filtrometre sistemi kullanıldı ve filt‐ rasyon basınç değerleri ölçüldü. Bulgular: Eritrosit deformabilite göstergesi olarak kullanılan filtrasyon basınç değeri gruplar arasında istatistiki olarak anlamlı farklılık gösterdi (p=0,02). C‐iskemi‐reperfüzyon grubundaki filtrasyon basınç değerleri, iskemi‐reperfüzyon grubuna göre anlamlı olarak düşük ölçüldü (µ (IQR) 2,0 (0,30) e karşılık 2,65 (1,5) p

Abstract

Objective: Ischemia‐reperfusion injury frequently occurs in cardiovascular diseases which has effects on both the exposed organ or the whole body. One of the most important reasons for this harmful effect on the organism is the deterioration of erythrocyte’s rheological properties. The erythrocyte deformability is one of the first stage of the deterioration. In our study, we aimed to investigate the non‐en‐ zymatic free radical scavenger and natural antioxidant l‐carnosine, which might have protecting effects on erythrocyte deformability in lower extremity ischemia‐reperfusion injury. Material and Method: Rats were randomized into 4 groups, each compounded 6 animals: 1) Ischemia‐reperfusion group, 2) Control group (C), 3) L‐carnosine control group (C‐C), l‐carnosine + ischemia‐reperfusion group (C‐ ischemia‐reperfusion). General anesthesia was applied with ketamine (90 mg/kg) + xylasine (10 mg/kg) before the operation. Midline laparotomy was applied to rats which would undergo ischemia reperfusion. Heparin (20 IU/kg) was given to all groups 30 minutes before the procedure. Infrarenal abdominal aorta was exposured. Lower extremity ischemia was initiated after placing an atraumatic microvascular clamp on the abdominal aorta. Following the ischemic period of 120 minutes, the microvascular clamp on the abdominal aorta was removed and reperfusion was pro‐ vided for 120 minutes. L‐Carnosine (intraperitoneal, 250 mg/kg) was given to groups 1 hour before the ischemia‐reperfusion procedures. In the C group, laparotomy and abdominal aorta dissection (240 minutes) were performed, but ischemia‐reperfusion was not created in these groups. While the rats were under anesthesia, they were sacrificed by blood extraction from heart. Constant flow filtrometer system was used to measure the deformability of erythrocytes.
