Özet

Amaç: Alzheimer hastalığı olan bir erişkinin temporal kemiğinde histolojik bir çalışma yapılmış ve anatomik detaylar, farklılıklar ve patolojiler ortaya konmaya çalışılmıştır.Çalışma Planı ve Yöntemler: Seksen dört yaşında solunum yetmezliğinden ölen Azheimer'li bir erkek hastanın sol temporal kemiği otopside çıkarıldı. Temporal kemik histolojik çalışma örneklerinin hazırlanmasında Schuknecht tarafından tarif edilen yöntem kullanıldı. Örnekler ışık mikroskobu altında incelendi.Bulgular: Otopsi sonucunda hastada orta derecede ilerlemiş Alzheimer hastalığı, ateroskleroz ve bron-kopnömoni bulundu. Temporal kemik incelemesinde, orta kulakta, oval pencerenin anterior nişinde minimal otosklerotik foküs ve Fallop kanalında oval pencerenin üzerinde dehisans saptandı. Diğer orta kulak bulgularının normal olduğu görüldü. Kokleada, Korti organı, stria vaskularis ve spiral ligamentte bütün kıvrımlarda yaşla orantılı dejenerasyon ve spiral ligament atrofisi görüldü. Spiral ganglion hücrelerinin bazal turn'lerde azaldığı gözlendi; üst turn'lerde hastanın yaşına göre normaldi. Vestibüler sistem bulguları normaldi. Sonuç: Orta derecede ilerlemiş Alzheimer'li bu olguda sadece yaşıyla orantılı histopatolojik presbikuzi bulguları saptandı.

Abstract

Design and Methods: The left temporal bone of a 84-year-old patient with Alzheimer's disease, who died from respiratory failure, was removed at autopsy. Preparation of the temporal bone for histologic study was made according to the method described by Schuknecht. Histologic specimens were examined under light microscopy. Results: Autopsy examination showed moderate Alzheimer's disease, moderate atherosclerosis, and extensive bilateral bronchopneumonia. Temporal bone examination showed minimal otosclerotic foci in the anterior oval window niche and dehiscence on the oval window of the Fallopian canal. Other middle ear findings were normal. Aging-related degeneration was noted in all turns of the cochlea, organ of Corti, stria vascularis, and the spiral ligament. There was atrophy of the spiral ligament. The number of the spiral ganglion cells were decreased in the basal turns, but appeared normal in relation to age in the upper turns. Vestibular system findings were normal. Conclusion: Histopathologic findings in this patient with moderate Alzheimer's disease showed only presbycusis attributable to aging.
