Özet

Amaç:Bezoarlar, nadiren intestinal obstrüksiyona neden olabilmektedir. Bezoarların preoperatif tanısı, onlara cerrahi tedavi dışında farmakolojik veya endoskopik tedaviler seçeneklerini sunabilir. Ancak bu konudaki literatür bilgileri sınırlıdır. Bu çalışmanın amacı kliniğimizde tedavi edilen 25 bezoar olgusunun derlenmesidir. Hastalar ve Yöntemler:2000 Eylül – 2007 Aralık arasında bezoara bağlı akut intestinal obstrüksiyon nedeniyle tedavi edilmiş 25 olgunun kayıtları retrospektif olarak değerlendirildi. Hastaların yaş, cinsiyet, geçirdiği operasyonlar veya sistemik hastalıkları, alışkanlıkları, ağız ve diş hijyeni, çiğneme fonksiyon bozukluğu, preoperatif görüntüleme yöntemleri, uygulanan tedavi ve operasyon bulguları, postoperatif komplikasyon bilgileri ve hastanede kalış süreleri değerlendirildi. Bulgular:Bezoar nedeniyle konservatif veya cerrahi tedavi uygulanan 25 olgunun 14’ü erkek 11’ i kadın iken yaş ortalaması 61 (23-82) idi. 19 (% 76) olguda geçirilmiş cerrahi öyküsü bulunmaktaydı (11 mide cerrahisi, 4 appendektomi, 2 histerektomi, 2 açık kolesistektomi ). 14 olguya abdominopelvik Bilgisayarlı tomografi (BT) çekildi. 12 olguda (%85) BT bezoar için diagnostikti. Tüm bezoar olgularımız başlangıç aşamasında konservatif yöntemlerle tedavi edilmeye çalışıldı. 4 olguya endoskopi yapıldı.3 olguda (%12) bezoarlar bulunup endoskopik olarak tedavi sağlandı. Konservatif yöntemlerle tedavi edilemeyen 14 hasta (%56) cerrahi ile tedavi edildi. Laparotomide 3 olguda bezoar midede, 6 olguda ileumda ve 5 olguda hem mide hem de ileumda bezoar saptandı . 10 hastada bezoarlar parmak ile ezilerek obstrüksiyon açıldı. Fragmantasyonun yapılamadığı 2 olguya ileotomi, 1 olguya gastrotomi, 1 olguya gastrotomiyle eş zamanlı ileotomi uygulanması gerekti. Postoperatif takiplerde 1 hastada yara yeri enfeksiyonu gelişirken, 1 olguda kardiyak sorunlar gelişti. Hiçbir olguda reoperasyon gerekmedi. Hastanede ortanca yatış süresi 5 (3 - 14) gündü. Sonuçlar:Bezoarlar intestinal obstrüksiyonun nadir sebepleri arasındadır. Mide cerrahisi geçirmiş ve intestinal obstrüksiyonu olan olgularda şüphelenilmesi gereklidir. Gereksiz gecikmeleri önlemek için bezoar şüphesi bulunan olgulara BT erken çekilmelidir. Endoskopi hem tanı hem tedavide kullanılabilir. Cerrahi tedavide tüm ince barsak ve mide ek bir bezoar olup olmadığı açısından kontrol edilmelidir.

Abstract

Purpose:Bezoars rarely can cause bowel obstruction. Diagnosis of bezoars preoperatively may allow pharmacological and endoscopical treatment choice instead of surgery. However, there is a limited information in literature, The present study aim to evaluate of 25 bezoars cases that treated in our clinic. Patients and Methods:Between September 2000- December 2007, a total of 25 consecutive cases managed for bezoars-induced acute intestinal obstruction records were reviewed. Age, sex, operation, or systemic disease history, habituations, mouth maintenance, chewing function, preoperative imaging, management, surgical approach and findings, postoperative complications, hospital stay were evaluated.Results:This study consisted of 14 men and 11 women with a mean age of 61 (23-82) years. A total of 19 patients (76%) had a history of previous surgery (11 gastric surgeries, 4 appendectomies, 2 hysterectomies and two cholecystectomy). Computer Tomography (CT) performed for 14 patients. CT was diagnostic for bezoars in 12 cases (85). All patients were initially treated conservatively. Endoscopy was performed for 4 cases. Bezoars were seen in 3 cases and treated endoscopically. Conservative management attempts failed in 14 cases, those were performed surgery. At laparotomy, bezoars were found in stomach in 3 cases, in ileum in 6 cases and both stomach and ileum in 5 cases. The bezoars could be fragmented in 10 cases. Three cases need enterotomy and bezoar extraction, and one case gastrotomy. Wound infection develop in one case and cardiac problem occurred in one postoperatively. None of the cases had relaparotomy. Median hospital stay was 5 (3-14) days. Conclusion:Bezoar induced intestinal obstruction remains an uncommon diagnosis. In intestinal obstructions it should be suspected in patients with previous gastric surgery history. In order to reduce unnecessary delays, CT should be performed early in these at-risk patients. Endoscopy can be used for both diagnosis and also treatment. During surgery all of the bowel and stomach certainly should be explored for additional bezoars.
