İdiopatik anal inkontinans tedavisinde periferik nöromodülasyonun etkinliği

Özet
Amaç: İdiopatik anal inkontinans için tedavi seçenekleri sınırlıdır. Sakral nöromodülasyon tekniği birçok çalışmada etkili bir tedavi seçeneği olarak önerilmesine rağmen kalıcı bir stimülatör yerleştirilmesi nedeniyle bazı dezavantajları da beraberinde getirmektedir. Bu çalışmada idiopatik anal inkontinans tedavisinde posterior tibial sinir stimülasyonunun etkinliğinin araştırılması amaçlanmıştır. Yöntem: Stimülasyon işlemi idiopatik anal inkontinans tanısı konulan 14 hastaya uygulandı. İnkontinans derecesi Wexner inkontinans skoru yanında anal inkontinans şiddeti ve hayat kalitesini değerlendiren inkontinans skorlamaları kullanılarak belirlendi. Herhangi bir anorektal cerrahi işlem geçirmemiş ve herhangi bir tedavi yöntemi uygulanmamış hastalar çalışmaya alındı. Hastalar prospektif olarak detaylı hikaye ve klinik muayene yanında endoanal ultrasonografi ve anorektal fi zyoloji testleri ile değerlendirildi. Stimülasyon 0.5-10 mA aralığında, 200 μs genişliğinde bir akım ile 20 Hz’lik frekansta tek tarafl ı olarak yapıldı. 4 hafta günaşırı 30 dakika süre ile yapılan stimülasyon işlemi bu dönem sonrasında 2 ayda bir olmak üzere 3 kez tekrarlandı. Stimülasyon sonrası hastalar en az 1 yıl süre ile takip edildi. Birinci yılın sonrasında anal inkontinans skorları ve anorektal fi zyoloji testleri tekrarlandı. Abstract
Purpose: Th e modes of management for idiopathic anal incontinence are very limited. Th e effi cacy of sacral nerve stimulation has been shown in some studies and has been the proposed route of management. However application of a permanent neurostimulator is one of the important disadvantages of the procedure. In the present study it was aimed to investigate the effi cacy of posterior tibial nerve stimulation in the treatment of idiopathic anal incontinence. Methods: Th e stimulation procedure is applied to 14 patients with the diagnosis of anal incontinence. Th e classifi cation of the severity of the incontinence was done using Wexner scoring system as well as other scoring systems measuring anal incontinence severity and the quality of life. Th e patients who have not undergone any surgical intervention or any other treatment protocol have been included in the study. Th e patients evaluated prospectively by anamnesis, physical examination together with endoanal ultrasound and other anorectal physiological tests. Th e stimulation is performed with a current 0.5-10 mA with a width of 200 μs and a frequency of 20 Hz. Th e procedure is performed every 2nd day for 4 weeks followed by once every 2 months for three times. Th e anal incontinence scoring and anorectal physiological tests were repeated at the end of fi rst year.
