Hemoroidektomiye bağlı anal stenoz tedavisinde V-Y anoplasti

Özet
Anal stenoz nadir görülen bir patoloji olup, epitelyum hattının farklı derecelerde fibröz bağ dokusuna dönüşerek anal kanalın anormal daralmasını ifade eder. Anorektal cerrahi ameliyatlarının komplikasyonu olup, çoğunlukla hemoroidektomi sonrası görülür. Anal stenozun etkili cerrahi tedavisi güçtür. Bu çalışmada hemoroidektomiye bağlı gelişen 2 anal stenoz olgusu ve cerrahi tedavide uygulanan V-Y anoplastiyi sunmayı amaçladık. Olgu 1: 45 yaşında erkek hasta. Grade 4 hemoroid nedeniyle 1 yıl önce LigaSure hemoroidektomi uygulanan hastada 3 ay sonra anal stenoz gelişti. Diyet tedavisi yanında 3 kez buji dilatasyonu ile yeterli sonuç alınamayınca tek taraflı V-Y anoplasti yapıldı. Olgu 2: 38 yaşında kadın hasta. Grade 4 hemoroid nedeniyle 10 ay önce LigaSure hemoroidektomi yapılan hastada 2 ay sonra anal stenoz gelişti. Diyet ve 2 kez yapılan buji dilatasyonu ile şikayetleri gerilemeyen hastaya tek taraflı V-Y anoplasti uygulandı. Postoperatif dönemde dışkılama güçlüğü, anal ağrı ve kanama şikayetleri geçen hastaların ortalama 6 aylık takiplerinde komplikasyon ve nüks görülmedi. Sonuç olarak; diyet ve dilatasyon tedavisine cevap vermeyen şiddetli anal stenoz tedavisinde V-Y anoplasti etkili bir cerrahi yöntemdir.
Abstract
Anal stenosis is a rare entity which is characterized by an abnormal narrowing of anal canal due to progressive fibrosis of anal epithelium. It is a complication of anorectal surgery where mostly encountered after hemorrhoidectomy. The effective treatment of anal stenosis is difficult. We aimed to present two cases of anal stenosis developed after hemorrhoidectomy and V- Y anoplasty for their surgical management. Case 1: 45- years old male patient underwent LigaSure hemorrhoidectomy 1 year ago for Grade 4 internal hemorrhoids. Three months later he developed anal stenosis. Since both dietary treatment and anal dilatation did not give effective results, patient underwent V-Y anoplasty. Case 2: 38-year old female patient underwent LigaSure hemorrhoidectomy 10 months ago for Grade 4 internal hemorrhoids. Anal stenosis which was refractory to dietary measurements and anal dilatation developed 2 months later. A one-sided V-Y anoplasty was performed to the patient. Anal pain, strained defecation and bleeding complaints of both patients disappeared postoperatively. There was neither any complications nor recurrence detected during 8 months follow-up. V-Y anoplasty is an effective method for anal stenosis which is refractory to dietary measurements and repeated anal dilatations.
