Pilonidal sinüs tedavisinde primer kapama basit ve etkili bir yöntem

Özet

AMAÇ: Bu çalışmanın amacı, pilonidal sinüs nedeni ile eksizyon ve primer kapama tekniği ile opere edilen hastaların kısa ve uzun dönem sonuçlarını ortaya koymaktır. YÖNTEMLER: Çalışma, prospektif olarak Vakıf Gureba Eğitim ve Araştırma Hastanesi, 3. Genel Cerrahi Kliniğinde 2008 Ocak ile 2010 Ocak tarihleri arasında yürütülmüştür. Eksizyon ve primer kapama yöntemiyle opere edilen 162 pilonidal sinüs tanılı hasta, dosya kayıtları ile birlikte incelendi. Hastaların yaş, cinsiyet,ameliyat bilgileri, hastanede yatış süresi, postoperatif erken dönem ağrı ve komplikasyonlar,yara iyileşme ve işe geri dönüş süreleri, insizyon bölgesindeki uzun dönem şikayetler, nüks gelişimi ile ameliyat memnuniyetleri değerlendirildi.BULGULAR: Hastaların 145’i (% 89.5) erkek, 17’si (%10.5) kadın idi. Yaş ortalaması 23.7, (15- 54 arası) değişmekteydi. Ortalama ameliyat süresi 21.48 dakika (15-45 dakika arası) idi. Hastanede ortalama yatış suresi 1.11 gün olarak belirlendi (1-4 gün arası). Postoperatif dönemde 20 hastada yara yeri enfeksiyonu (%12.3), 2 hastada hematom (% 1.2), 2 hastada seroma (%1.2) gelişti. Yara ayrışması tesbit edilen 4 hasta (%2.5) pansuman ve antibiotik tedavisi ile takip edildi. Hastaların takip süresi ortalama 12 ay ve 2- 24 ay arası idi. Bu takip dönemlerinde 12 hastada nüks tesbit edildi. Çalışmada nüks oranı %7.4 olarak bulundu. Hastaların 86’sı ameliyatı mükemmel, 24’ü iyi, 46’sı orta ve 6’sı kötü olarak değerlendirdi.SONUÇ: Primer kapama, postoperatif minimal ağrı ve erken yara iyileşmesi ile kolaylıkla uygulanan bir tekniktir. Ayrıca erken işe dönüş ve düşük nüks oranlarına sahiptir. Teknik bu avantajları ile pilonidal sinüs cerrahisinde tercih edilebilir.

Abstract

OBJECTIVE: The objective of this study is to determine the short and long term results of pilonidal sinus after excision and primary closure.METHODS: The study is conducted prospectively at Vakıf Gureba Training and Research Hospital, 3. General Surgery Department between January 2008 and January 2010. The medical records of 162 patients operated due to pilonidal sinus with primary closure technique were studied. Collected data about patient’s age, sex, operation details, duration of hospitalization, postoperative pain and complications, wound healing time and time to return to work, complaints on incision site and recurrence rates were evaluated.RESULTS: There were 145 (89.5 %) men and 17 (10.5 %) women with a mean mean age of 23.7 years (range, 15-54 years). Mean operation time was 21.48 minute (range, 15-45 minute). Mean duration of hospitalization was 1.11 day (range, 1-4 day). There were wound infection in 20 patients (12.3%), hematoma in 2 patients (1.2%) and seroma in 2 patients (1.2 %).Four patients with wound dehiscence were managed with dressing and antibiotheraphy. The mean follow-up time was 12 months (range, 2-24 months). The recurrence rate was 7.4%. The operation result was excellent for 86 patients, good for 24, fair for 46 and bad for 6 patients. CONCLUSION: The primary closure is a simple technique with minimal postoperative pain and good wound healing. It has also other advantages such as early return to work and low recurrence rates. It can be prefered method in pilonidal sinus surgery.
